This notice / solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76. THIS NOTICE CONSTITUTES THE ONLY SOLICITATION. This requirement is solicited on an unrestricted basis. The Marine Corps Logistics Command (MCLC) Albany, Georgia intends to solicit and award a Firm Fixed Price contract for this requirement. The North American Industry Classification System (NAICS) 811310 - Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenance; Size Standard $7.5M applies. The government reserves the right to make one, multiple or no award resulting from this solicitation. Notes: 1. Performance Work Statement is attached below. 2. Required service location: On-Site Marine Corps Logistics Base Barstow, CA 92311. Base Year CLIN 0001: On-Site Scheduled Preventative Maintenance Service of (one) Flow International, Electric Husky Water Blast Machine model E-150 and Jet Lance model A-3000S. 365 day period of performance. See Performance Work Statement attached below. Option(s) CLIN 1001: On-Site Scheduled Preventative Maintenance Service of (one) Flow International, Electric Husky Water Blast Machine model E-150 and Jet Lance model A-3000S. 365 day period of performance. Contractors must be registered in the Central Contractor Register (CCR) and Wide Area Workflow (WAWF) to be considered for award. The United States Marine Corps utilizes WAWF as its ONLY authorized method to electronically process vendor requests for payment. All quotes must be received by the above listed Point of Contact (P.O.C), no later than the Solicitation Closing Date indicated above. Attachment I PERFORMANCE WORK STATEMENT •1. Scope : The required Preventive Maintenance (PM) Service is to include all labor, standard maintenance parts and travel related expenses incurred to maintain the government owned Flow International - Electric Husky Blast Machine, model E-150 and Jet Lance, model A-3000S on-site aboard Marine Corps Logistics Base Barstow, California. •2. Applicable Documents attached : Flow International Electric Husky E-150 Technical Manual M-277 Rev. E October 2007 Flow International Jet Lance A-3000S Technical Manual M-279 Rev. K November 2009 3. Requirements: Preventative Maintenance shall be performed in accordance with the Original Equipment Manufacturers (OEM) technical manuals attached and specified in this document. The Vendor performing this service shall maintain the equipment for a base period of performance of one year requiring six on-site visits. The contract shall also include three (3) one-year option periods. Normally consumed spare parts, lubricants and materials shall be included in the contract price and installed/consumed during the scheduled maintenance visits. The following maintenance shall be performed as a compliment to the perscribed services outlined in the OEM technical manuals: •1) A bi-monthly PM to include testing and evaluation and necessary service of all equipment with recommendations for replacement parts nearing or at the end of their useful life. To include: •a. Six (6) Bi-Monthly Preventive Maintenance Visits •b. The on-site service performed shall include the 200 hours service on the pump and the A-3000S hand tool service. •c. The sixth visit shall include the bi-monthly services described above and the installation of: •- New Check Valves •- New high pressure cylinders •- New bearing carriers •- New sub-plate adapters •- Inspection of end caps •- Inspection of sub-plate •d. Any and all updates to equipment shall be conducted once notified by manufacturer's specifications. •e. Maintenance of water treatment system •f. All labor and travel related expenses shall be included in the unit price 4. Installation Safety requirements : These requirements apply to contractors, subcontractors, and their employees or agents. Within one (1) hour of any mishap or incident resulting in personal injury (OSHA recordable) and/or significant property damage while performing work authorized by this contract, the contractor shall notify the following: •1. The activity's Safety Representative - Name/Phone Number: Maria Ramirez (760)577-7281; •2. The COR (if applicable); •3. The Contracting Officer; •4. LOGCOM Installations, Environment, and Safety (IE&S) office. A written report of the mishap/incident shall be sent within 24 hours to IE&S/S11. The Government point of contact is Kathy Berry at 229-639-6284 or email kathy.berry@usmc.mil. Mishap notifications shall contain, as a minimum, the following information: •1. Contract Number, Name of Company, and Name and Title of Person(s) Reporting •2. Date, Time, and exact location of accident/incident •3. Brief narrative of accident/incident (Events leading to accident/incident) •4. Cause of accident/incident, if known •5. Estimated cost of accident/incident (material and labor to repair/replace) •6. Nomenclature of equipment and personnel involved in accident/incident •7. Corrective actions (taken or proposed) •8. Other pertinent information Contractor must fully cooperate with LOGCOM safety personnel during safety inspections of the workplace and inspections of required documentation. Safety requirements listed in this package that do not relate to the contractor's operations or services may be waived by the Contracting Officer. Objections made by the contractor must be reported to the Contracting Officer within 10 days of contract award. Objections will be considered on a case-by-case basis. Smoking and the use of tobacco products is allowed in designated areas only. All vehicle operators and passengers of contractor and privately owned vehicles are required to wear seatbelts while their vehicle is in operation on the installation and/or performing work under this contract. The use of seatbelts is also required on all mobile construction equipment operating on the installation and/or performing work under this contract. All vehicle operators and passengers are prohibited from using a cell phone, personal digital assistant (PDA), or Blackberry TM, unless the vehicle is safely parked or the operator is using a hands-free device. The use of portable headphones, earphones, or other listening devices (except for hand-free cell phones) while operating a motor vehicle is also prohibited. Should an accident occur and i n the event the US government investigates the mishap, the contractor and subcontractors shall cooperate fully and assist government personnel during the investigation period. Instructions pertaining to the assistance required from the contractor in support of an investigation and the point of contact of the LOGCOM safety personnel involved in an investigation will be provided by the COR or the Contracting Officer. Contract Labor Hour Reporting: The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the [NAMED COMPONENT] via a secure data collection site.
