Description
In accordance with the terms of FAR 12.603(c): (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is RFQ-NIH-NEI-3532648 and is issued as a Request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76. (iv) This is a FULL AND OPEN COMPETITION under NAICS 339112 with a size standard of 500 employees. (v) Please see requirements document for the specs of the sterilizers. (vi) Please see attachment for description. (vii) FOB origin, within 5 months of order to 9000 Rockville Pike, Bethesda MD. (viii) The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition with no addenda. (ix) The evaluation criteria at 52.212-2 do not apply. The evaluation criteria can be found in the attachment.. (x) Please include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. On online version maybe reference in its place. (xi) The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. See attachment. (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items. The applicable clauses are selected in the attachment. (xiii) No other contract requirements at this time. (xiv) A statement regarding the Defense Priorities and Allocations System (DPAS) is not applicable. In spite of not opening a question period, the government felt it was important to provide the information requested. 1. Is this subject to new / unused equipment only Turnkey project. 2. The RFQ calls for: Installation will include removal and disposal of existing equipment, receiving, uncrating, rigging, set-up and level, assembly, utility connections. a. Will a site visit be available? No b. If not, please provide the following information i. What floor does the unit go to? B1 ii. Is someone available to receive the unit? The unit will need to be received by the contractor with coordination of the loading dock personnel iii. Measurements of hallway, doorways. Smallest hallway is 48 wide ” and smallest door is 41 ½ wide” iv. Measurement of elevator, weight limitations? 7 feet wide and 8 feet high. 8,000 lb load capacity. 4. Is there an existing unit to be un-installed? a. If so who is to dispose of unit Yes, and it will be the responsibility of the contractor. 5. What utilities will be available? a. Water pressure Facility water supplied at 30-50 psig at 15 gpm @ 70oF, ¾”NPT. b. Steam pressure Facility trapped steam supplied at 40-80 psig at 1,200 lbs/hr, 1½” - 2”NPT. c. Floor drain 10”floor sink with 3” drain or 6-inch open facility floor drain. d. Electric 120V / 1 phase / 60 Hz / 5 amps e. Required shut-offs provided by customer
