THIS IS A SOURCES SOUGHT ANNOUNCEMENT FOR INFORMATION ONLY. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT WILL BE AWARDED FROM THIS ANNOUNCEMENT. In preparation for a future procurement, G.V. Montgomery VA Medical Center in Jackson Mississippi is performing market research to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned, small business, or large business) relative to NAICS 811219 that can offer the services listed below: STATEMENT OF WORK Transaction Number: 586-15-1-033-0022 1.EQUIPMENT IDENTIFICATION: ENTRY #MANUFACTURERMODELSERIAL #LOCATION ALCONINFINITI VISION SYSTEM OZIL0602424101X 1302332801X RM; L-17 RM; L-14A 2. SERVICE/ EQUIPMENT The G.V. (Sonny) Montgomery VA Medical Center, in Jackson, MS is seeking a contractor to furnish all labor, materials, transportation, tools, supervision and software/hardware upgrades to provide preventative maintenance and full corrective service of government owned equipment; 2 each Infiniti Vision System Ozil, located at G.V. (Sonny) Montgomery VAMC, 1500 E. Woodrow Wilson Jackson, MS 39216, for the period of October 1, 2014 through September 30, 2019 (includes base year and four (4) option years). Equipment shall be in good operating condition upon the effective date of this contract. GVSMVAMC has the Infiniti Vision Systems Ozil and service contract for technical support is required. Contractor shall provide maintenance support, corrective maintenance repair, 24x7 emergency phone support, parts and service, and modification due to safety or reliability. Period of Performance: Oct 1, 2014 through Sept 30, 2015 (Base Year) Definitions: COR: Contracting Officer's Representative GVSMVAMC: G.V. (Sonny) Montgomery Veterans Affairs Medical Center VHA: Veterans Health Administration VA: Department of Veterans Affairs VA Mission: "To fulfill President Lincoln's promise "To care for him who shall have borne the battle, and for his widow, and his orphan" by serving and honoring the men and women who are America's veterans." 1.TASKS: Requirements: Performance: If support call is deemed mission-critical, contractor will be on-site performing corrective action response to service calls within 48 hours. All work is to be performed by competent personal, experienced, and qualified to work on equipment. All work performed shall be in accordance with manufacturer's instructions, including but not limited to: adjustments, calibrating, cleaning, lubricating, testing, disassembly, checkout, replacement of parts, etc; required to keep the equipment in operating condition. Contractor shall guarantee that all equipment covered by this contract shall be in optimum working condition at the expiration date. Communication Services: Service Call: Unscheduled service should be conducted at no additional cost between the hours of 8am and 5pm, Monday through Friday. Preventive maintenance calls should be scheduled with Biomedical Engineering between the hours of 8am and 5pm. Contractor shall provide 1 preventive maintenance visit per year. Help Desk Support: Remote services, and technical and clinical telephone support will be provided 24x7. Upgrades: Software and hardware updates for reliability and performance enhancement shall be included. All parts to provide maintenance shall be included. One lamp replacement shall be included each year. Contractor Qualifications: Contractor personnel shall be trained and qualified to work on all components of the microscopes. Contractor personnel shall provide proof of pertinent successful completion of training and/or certification, upon customer inquiry. Also, firms should provide their business size status in accordance with the small business administration website (www.sba.gov) for the above referenced NAICS code they fall within (i.e. small businesses, 8(a) firms, HUB Zones, small disabled veteran owned businesses, etc.) This Sources sought is for informational purposes only and all businesses able to provide this equipment are encouraged to submit information regardless of Business size.
