***The purpose of this modification is to correct the NAICS code from 423460 to NAICS code 339115.**** The Department of Veterans Affairs intends to negotiate sole source commercial item procurement to Dutch Ophthalmic US. Dutch instrumentation and Vitrectomy lenses are used for ophthalmic surgery. The Dutch instrumentation and Vitrectomy lenses are compatible with the Dutch 6000 Dual Vitrectomy System which was recently purchased by Fayetteville VA Medical Center Ophthalmology Service located at 2300 Ramsey Street, Fayetteville, NC 28301, which consists of: Dutch Ophthalmic: Instrumentation and Vitrectomy lenses for Vitrectomy procedures "Microforceps: end-gripping, 23g "Microforceps: serrated jaws, 23g "Microforceps: Shah micro-serrated jaws, 23g/0.6mm "Microscissors: Vertical, 135 degrees, 23g "Microforceps: 23g Diamond Dusted "Microscissors, cvd, horizontal, 23g "Kitaoka PVD Spatula 23 gauge/ 0.6mm "Eckardt Membrane Pic, 23g "23g Endodiathermy Needle "Cable for Intraocular electrodes (1120, 1121, 1122) "Eckardt Backflush Handpiece, 23g, passive aspiration "Large Replacement Backflush Reservoir (10/Bx) "Backflush needle with blunt 20g needle, passive aspiration "WideView Vitrectomy Lens: flat lens "WideView Vitrectomy Lens: biconcave lens "WideView Vitrectomy Lens: wide field "WideView Vitrectomy Lens: 20 degree prism lens "Wideview Vitrectomy Lens: 30 degree prism lens "Vitrectomy Lens Ring Trade in unit: None Delivery to be performed within 1 year of award. This proposed contract action is for supplies or services for which the government intends to solicit and negotiate with only one source under the authority of FAR 6.302(b)(1)(i). This notice of intent is not a request for competitive quotes. However, interested parties may identify their interest and capability to respond to the requirement and submit quotations no later than 4:00 EST, September 22, 2014 EST to: danielle.may2@va.gov or Fax: 757-728-3132. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
