Description

AGENCY: Veteran's Health Administration, Department of Veterans Affairs Kevlar Patient Immobilization Kit, VA, VISN 3 - NYH Brooklyn Campus, NY THIS SOLICITATION NOTICE IS IN REFERENCE TO RFI# VA243-14-I-1703 THAT WAS POSTED ON 9-4-2014 ON FBO. A FORMAL SOLICITATION DOCUMENT WILL BE POSTED DIRECTLY ON FBO.GOV. THIS IS AN UNRESTRICTED/ FULL & OPEN PROCUREMENT. The Brooklyn VAMC located at 800 Poly Pl Brooklyn, NY 11209 requires one Brand Name or Equal SBRT Kevlar Patient Immobilization Kit with Platform Components and quantities detailed in the solicitation document. Responses to this notice must be submitted in writing (via E-mail) referencing the requirement & RFQ # and must be received not later than September 25, 2014 at 11:00AM EST. Email: lawrence.unger@va.gov. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, description of item(s) available, DUNS number, address, point of contact and social-economic status verification. To be eligible for consideration, offerors should have proof of successful delivery of same or like items in the past, availability of item(s) currently in stock, and be capable of presenting certification of such with verifiable references.
