Description

This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in FAR 12.6, as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. This is a Request for Quote (RFQ) and the solicitation number isVA248?14?Q?2029. This acquisition is open market and a set aside for Service?Disabled Veteran?Owned (SDVOSB) concerns. The C.W. Bill Young VAMC, formally known as Bay Pines VAMC, anticipates the award of a firm fixedprice contract to manage the hazardous and non?hazardous waste in the Bay Pines Veteran Healthcare System (BPVAHS) and its associate facilities located throughout the southwestern region of Florida. This contract will consist of a one base year and one option year. A quotation in response to this notice must be received not later than 12:00pm EST Tuesday September 25, 2014. The solicitation number is VA? 248?14?Q?2029 and is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005?056. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the Offerors responsibility to ensure that the quote is received. The applicable North American Industry Classification System Code (NAICS) is 562112, Hazardous Waste Treatment and Disposal; the size standard is 32.5 million. OFFERORS MUST STATE THEIR BUSINESS SIZE when submitting their offer. DESCRIPTION OF REQUIREMENTS ***All bidders must acknowledge the most recent amendment to the Statement of Work (SOW) for solicitation VA248?14?Q?2029 posted 09/19/2014 by emailing Contract Specialist, Dwyane Tyree (dwyaneallen.tyree@va.gov) by 4:00pm EST Monday September 22, 2014 confirming the changes. Potential bidders who do not acknowledge the changes to the SOW by the aforementioned date and time will be disqualified from consideration for award. ACKNOWLEDGMENT OF AMENDMENTS: The offeror acknowledges receipt of amendments to the Solicitation numbered and dated as follows: AMENDMENT NO DATE A00001 09/22/2014 The Contractor shall provide all supplies, parts, materials, equipment, labor, supervision, management and transportation to furnish emergency and routine solid and regulated waste management and disposal. Although a specific waste disposal pricing list will be generated by the Contractor, additional wastes may be required to be included at a later time, as needed by BPVAHCS. Detailed description of the location and requirement involved is on the purchase order and furnished to all bidders. Solicitation number VA248?14?Q?2029. DELIVERY ADDRESS A service contract is required to manage the hazardous and nonhazardous waste in the Bay Pines Veteran Healthcare System (BPVAHS) and its associate facilities located throughout the southwestern region of Florida.
