Description

This is a solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-14-Q-0739 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 811219. This is a small business set aside. All responsible sources may submit a proposal which shall be considered by the contracting officer. SERVICE REQUIREMENTS: Tissue Processor Full Maintenance Service Contract FY 15 (Serial Number EX26430803, Manufacturer: Richard Allan Scientific) Contractor shall provide all supervision, labor, tools, parts, materials and travel required to perform all needed preventive maintenance and any needed corrective maintenance of the Tissue Processor (Serial Number EX26430803, Manufacturer: Richard Allan Scientific) at the Department of Veterans Affairs, Northern Indiana Healthcare System (NIHCS), 2121 Lake Avenue, Fort Wayne, IN 46805 for the period 10/1/14 - 9/30/15. Offers shall be submitted to Kellie Konopinski, Contract Specialist, at Kellie.Konopinski@va.gov by no later than Thursday, September 25, 2014 at 11:00 am, EST.
