Description
Base Pass Template FA4861-14-R-C001 Attachment 5-Past Performance Questionnaire-1 Attachment 4-WD 05-2331 (Rev.-14) Attachment 3-Pricing Table Attachment 2- EIPS 30 JULY 2014 Attachment 1- PWS Nellis-Creech Sept 2014 This is a solicitation for "Base Telecommunications Systems" for Nellis/Creech AFB, Nevada. This is a commercial service; therefore, FAR Part 12, Acquisition of Commercial Items, shall apply to this acquisition. The Contractor shall provide all personnel, equipment, tools, supervision, and other items and services necessary to ensure that BTS O&M are performed at Nellis Air Force Base in accordance with the Performance Work Statement (PWS). All work performed by the contractor shall be performed in accordance with all applicable laws, regulations, standards, instructions, and commercial practices. This acquisition will be solicited as a Request for Proposal (RFP) utilizing FAR Part 15, Contracting by Negotiation. A single award Firm Fixed Price, Performance Based Indefinite Delivery/Indefinite Quantity (IDIQ) contract will result from the solicitation. Award selection will be based on Lowest Price Technically Acceptable (LPTA) in accordance with Federal Acquisition Regulation (FAR) 15, specifically 15.101-2. Period of performance will be for a base period and two (2) six month option periods plus a six (6) month extension period (if needed). This acquisition is being solicited as a 100% Small Business Set-Aside. The North American Industry Classification System Code 517110 will apply to this acquisition; and, the small business size standard is 1500 Employees. No paper copies will be issued; no telephone requests will be honored and no bidder's mailing list will be maintained. It is anticipated that the following viewers will be needed to view the solicitation and all attachments: Microsoft Work (.doc), Microsoft Excel (.xls), Adobe Acrobat reader (.pdf) and WinZip (.zip or.exe.). This viewer list is not all inclusive as other viewers may be required. To be eligible for contract award, all prospective offerors must be actively registered in the System for Award Management (SAM) database. The website for SAM is (http://www.sam.gov) and the SAM customer service telephone number is 1-866-606-8220. Registration requires applicants to have a Data Universal Numbering System +4 (DUNS+4). SAM registrations shall be current. All responsible small business concerns may submit a proposal to the 99th Contracting Squadron. A site visit is scheduled to take place on Tuesday, 7 October 2014, starting at 8:00 AM (Local NV Time) ; All contractors (Perspective Bidders/Offerors) will meet at the parking lot outside of the Main Gate; (located off of Nellis & Craig). THERE WILL BE A MAXIMUM OF THREE REPRESENTATIVES FROM EACH COMPANY ALLOWED TO ATTEND THE SITE VISIT. All contractors (Perspective Bidders/Offerors) must RSVP and will be required to submit names directly to SSgt Derek Urban, not later than Thursday, 30 September 2014, 2:00 PM (local NV Time). Use the provided excel spreadsheet entitled " Base Pass Template ". The following instructions apply: 1) Re-name the file to include the name of your company. 2) Inside the spreadsheet there are eight blocks that need to be filled out (Per attendee) - these blocks are highlighted in yellow. i) Company Name ii) Individuals Name (Last, First, Mi) iii) Country of citizenship iv) Date of Birth iv) Social Security Number iii) Driver's license # and state of issue 2) Submit the filled out Base Pass Template (excel document - NOT a scanned copy) - Via E-mail; to (derek.urban.1@us.af.mil) 3) Any questions pertaining to this acquisition shall be asked, in writing, no later than Tuesday, 14 October 2014, 11:00 AM (Local Nevada Time). Questions submitted after the aforementioned cutoff date will not be reviewed or answered. Questions shall be sent to SSgt Derek A. Urban via email at derek.urban.1@us.af.mil and TSgt Marico Gray at Marico.Gray@us.af.mil ***Failure to provide the required information outlined above within the stipulated time lines, will preclude the perspective bidders'/offerors' ability to gain access to Nellis Air Force Base, resulting, in the inability to attend the site visit. **Individual base passes will not be granted for personnel who arrive the day of the site visit without prior coordination.
