- SINGLE SOURCE 1.Contracting Activity: NCO 12, GREAT LAKES ACQUISITION CENTER, 115 SOUTH 84TH STREET, SUITE 101 MILWAUKEE, WI 53214 2.Purchase Request Number: 578-14-4-116-0136 3.Authority for Action: 1 FAR 13.106-1(b), Soliciting from a Single Source 4.Describe the supply or service to be acquired and the total estimated dollar value: The Research Service at Edward Hines Jr. VAH has a requirement for a psychoneuroimmunologist to oversee laboratory analysis of biomarkers related the the clinical mindfulness based stress reduction as outlined in Dr. Saban's VA funded grant. Total Estimated Dollar Value: $64,308.00 ($16,077.00 base year, plus three 1-year options). 5.List the proposed contractor (name, contact, and phone number) and describe the unique knowledge or capabilities of the proposed contractor: Herbert Mathews Consulting is uniquely qualified to provide oversight of laboratory services based upon their extensive experience and knowledge related to analysis and interpretation of biomarkers associated with stress reduction intervention. 6.Describe your market research efforts and efforts to ensure that offers are solicited from as many potential sources as is practicable: Market research found that the lone way the required services can be obtained is by Herbert Mathews Consulting, as shown in Dr. Saban's VA funded grant. 7.State why you feel that the anticipated cost to the Government will be fair and reasonable: IAW FAR 13.106-3(a)(2), the Contracting Officer determines the price to be fair and reasonable based on comparison with prices found reasonable on previous purchases. Same services were previously awarded with contract VA69D-13-P-5078 for $16,077.00 per year, to Herbert Mathews Consulting. It is anticipated the contractor will hold pricing the same, $16,077.00 per year, from the previous contract in 2013 for the same services. This price is consistent with previously-awarded contracts. 8.Other: Funding has been approved and awarded by VA Central Office, by the Director of Health Services Research based on proposal submitted by Dr. Saban, specifying Herbert Mathews Consulting under the direction of Dr. Mathews.
