Description
Supplier Data Sheet and Business Size Certification Form WV-0150 This is a sources sought announcement only. This is not a solicitation and proposals are not being requested at this time. The purpose of this announcement is to gain knowledge of current market capabilities. Eligible companies shall be located within the United States and U.S. owned. Interested parties should complete and submit Form WV-0150, "Supplier Data Sheet and Business Size Certification Form" to Joseph.Kwiatkowski@chbwv.com Interested companies should submit by 5:00 PM on October 14, 2014 their expression of interest (including Form WV-150) in receiving any resulting Request for Proposal (RFP) to the following email address: Joseph.Kwiatkowski@chbwv.com Please contact Joseph Kwiatkowski at (716) 942-4966 or at the above email address with any questions. The proposed Scope of Work is to provide CH2M HILL B&W West Valley, LLC (CHBWV) the following: 1. The Radiological Asbestos Laboratory (Lab) shall furnish the materials, equipment, facilities, personnel services, disposal, and other necessary and incidental items except as otherwise provided by CHBWV for the performance of asbestos analyses. Materials shall include provision of sampling media for the requested analysis. 2. Analyses to be performed will have priority levels as follows: i. Priority 1 - verbal/fax results, same day, within ten (10) hours of sample receipt, written report in three (3) working days ii. Priority 2 - verbal/fax results within twenty-four (24) hours of sample receipt and written report within three (3) working days iii. Priority 3 - written report within ten (10) working days. 3. The estimated number of annual samples and analyses would be provided in any resulting request for proposal. Primarily, bulk asbestos samples will be requested as Priority 3, and asbestos air samples will be requested as Priority 1. Periodically, different priorities may be required. 4. The laboratory shall be capable of analysis seven (7) days a week. 5. Samples that may be radiologically contaminated. Radiologically contaminated samples may contain Strontium 90, Cesium 137, Americium 241, Uranium 235, Uranium 238, Thorium 232, or Plutonium 239. The activity of the sample shall not exceed the maximum quantity that the laboratory may possess per sample. The laboratory shall submit their most current radioactive material license to CHBWV (in response to any requested RFP). 6. The Lab shall use the latest revision of NIOSH, EPA, and OSHA analysis. Substitution of equivalent methods is acceptable, subject to CHBWV review and written approval. 7. Equipment used in the processing, storage and analysis of samples from CHBWV shall be controlled. Calibration of analytical instrumentation shall be maintained and documentation of calibration status available to CHBWV. 8. NIST traceable standards shall be used. Standards and reagents shall be controlled and labeled as to expiration dates. Stock solutions prepared from such standards and reagents shall be traceable to a particular lot. Non-expired, reagent grade chemicals shall be used in all analysis. 9. To maintain chain-of-custody, the Lab will store samples in secure areas with controlled access. A sample "lock up" facility or a storage area in a controlled access laboratory would comply with this requirement. 10. Lab Quality Assurance Program shall satisfy the requirements of ASME NQA-1 (2008 with 2009 Addenda). NQA requirements 1, 2, 4-18. The Lab shall be accredited by New York State Department of Health ELAP approved for asbestos analyses for analysis of workplace environmental samples. 11. The Lab shall demonstrate (in response to any requested RFP) evidence of satisfactory analytical performance on crosscheck samples from state or national laboratory inter-comparison programs. Results of any such performance evaluations shall be sent to CHBWV with any resulting RFP. Quality control samples of known content provided to CHBWV by national laboratories will be submitted periodically to the Lab as blind samples. Failure to perform adequately on these known samples may be basis for a Request for Correction Action.
