Description
STATEMENT OF WORK A. GENERAL GUIDANCE 1. The James A. Haley VA Pharmacy Department at 13000 Bruce B. Downs Blvd. Tampa, Florida requires accreditation as a recognized provider of Pharmaceutical Education by boards of pharmacy that will count toward re-licensure in all 50 state and territories of the United States of America. 2. Background: The James A Haley has been a recognized provider of pharmaceutical education of over 20 years. Part of the mission of the pharmacy is to provide pharmaceutical education to the staff of the James A Haley VA, and also at times, the surrounding community and other VAs in the VISN 8 network. Pharmacist in this department practice under pharmacy licenses issued from state boards of pharmacy that are all over the Unites States. Accreditation is needed to provide a basis for uniform acceptance of continuing education credits among all the state board of pharmacy for bi-annual re-licensure. 3. Scope of Work: "The contractor shall provide James A. Haley Pharmacy Department accreditation as a provider of continuing pharmacy education. "The contractor's accreditation status must be recognized by boards of pharmacy in all 50 state and territories of the United States as continuing education that will count toward re-licensure. "Contractor must relay continuing education activities to the National Board of Pharmacy (NABP) after receiving the NABP e-Profile ID and Partial Birthday (Mo/Day) for every attendee of an educational program that meet the standards of accreditation. "The contractor will grant provider status to James A. Haley Pharmacy Department as long as the account is paid and uphold the standards of quality the contractor sets forth to attain provider status. 4. Performance Period: Contract will be used to purchase accreditation for pharmaceutical education provider status, Base Year - January 01, 2015 - December 31, 2015 with 4 option years. 5. Type of Contract: This is a firm-fixed-price agreement. B. GENERAL REQUIREMENTS 1.The contractor shall provide James A. Haley Pharmacy Department accreditation as a provider of continuing pharmacy education. 2.The contractor's accreditation status must be recognized by boards of pharmacy in all 50 state and territories of the United States as continuing education that will count toward re-licensure. 3.The Contractor will have a web based tool to aid in the collection of accredited education related activities for reporting purpose to NABP and continuing pharmacy education (CPE) tracking service (CPE Monitor). 4.The contractor will provide web based education when educational standards change. 5.Fee should be based on the number of pharmacy education credit hours granted by James A Haley Pharmacy Department. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES 1.Must keep James A Haley Pharmacy Department accredited provider status current 2.Must report all accredited continuing pharmacy education activities for each participant to NABP. 3.Continuing pharmacy education accreditation must be recognized by boards of pharmacy in all 50 state and territories of the United States. E. SCHEDULE FOR DELIVERABLES James A Haley provider accreditation status must be available on 01/01/15. G. CHANGES TO THE STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. H. QUALIFICATION/CERTIFICATIONS The contractor shall be qualified and have the required levels of professional and technical experience to perform the work required by this statement of work. I. SECURITY The C&A requirements do not apply and a Security Accreditation Package is not required. No patient or employee information that is considered sensitive will be exchanged with the contractor.
