- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Send Quote Packet as outlined in Item C below by email to Mr. Dennis C. Tremethick Dennis.C.Tremethick@usace.army.mil. Include your DUNS Number and CAGE Code in your email. If you Do not have a DUNS or CAGE, please register at www.sam.gov This is a Request for Quote (RFQ) for a competitive, firm fixed-price service contract with a duration of approximately two (2) months. The Contractor shall conduct the work described in the Statement of Work (SOW) independently; however, it is subject to inspection by the Project Manager under the Quality Surveillance Inspection Program (QASP). Enclosed you will find the Statement of Work (SOW) dated 09 October 2014, a Pricing Sheet, a Question and Answer (Q&A) Form, and Form SF1449 Solicitation / Contract / Order for Commercial Items. Also Attached are instructions to register in www.sam.gov. The award will be made based on the lowest priced Quote, which also meets or exceeds the acceptability standard for the non-cost evaluation factors as determined by the Project Manager. Price Evaluation is based upon the Total Quoted Price. A. Non-Cost Evaluation Factors: The offorer must furnish the following non-cost evaluation factors: i. Specialized Capability: The contractor must demonstrate experience and capability to assemble and disassemble modular office cubicles in a timely manner. This includes basic disassembly/assembly, connecting cubicle spaces, and installing wiring as required. ii. Locality: The contractor demonstrate that its labor and equipment are located in a place within a 2-hour, one-way, driving distance from the USACE San Francisco District Offices, located at 1455 Market Street, San Francisco, CA 94103. iii. Equipment and Labor: The contractor shall demonstrate it can provide adequate labor and equipment to complete the tasks as outlined in the Statement of Work within the timelines presented. iv. Experience: The contractor shall demonstrate its experience careful handling and hauling large quantities materials. (Provide at least two (2) examples that include duration of project, average volume of each load.) v. Explanation: The Contractor shall provide a detailed breakdown of how the company plans to fulfill the requirements as outlined in the Scope of Work. vi. Insurance: The Contractor shall provide evidence of active Liability Insurance. B. Site Visit: The San Francisco District Logistics Office will hold a site visit on Thursday, 16 October 2014, from 0900 to 1200 hrs at the SPN District Offices located at 1455 Market Street, San Francisco, CA 94103. If you wish to participate, you MUST CALL the Point of Contact, Chief of Logistics Boonchan (Mike) Pornnang, at tel. 415-503-6970 or email Boonchan.Pornnang@usace.army.mil before 12:00 PM (noon) on 15 October 2014. This is REQUIRED to provide your representative with an access pass into the building. There is no free parking in the building or in the local area. MR. PORNNANG WILL MEET THE GROUP IN THE LOBBY BETWEEN 8:50 AND 9:00 AM ON 16 OCTOBER 2014 AND ESCORT THE PARTICIPANTS TO THE APPROPRIATE LOCATIONS. Purpose of this site visit is to illustrate potential offerors the worksite and expectations in accordance with the Statement of Work. No agreements or changes to the Statement of Work will result from this visit without a formal Amendment to the Request for Quote signed by a Contracting Officer. B. Questions: Submit all questions in writing to Mr. Dennis Tremethick via email only no later than 4:00 p.m. (PDT), on 17 October 2014. All submitted questions and answers will be provided to all offerors via email. C. Submission Date: Please submit your price and non-cost factor answers by 4:00 p.m. (PDT), on 24 October 2014. Submit your Quote Packet; which includes your non-cost evaluation information, the completed signed SF1449 (including signed cover page), and the completed Pricing Sheet. All Quotes received by the official due date and time above with the non-cost factor information will be considered for the project.
