Tax Exempt Letter Sam.gov Registration Guide Vendor Packet EEO AA Non-Segregated Davis Bacon REQUEST FOR PROPOSAL Elevator Maintenance The Brooklyn Job Corps Academy is soliciting bids for preventive maintenance services for their elevators: • Automatic Passenger Elevator #3P7352, and • Freight Automatic Elevator #178665 The Brooklyn Job Corps Academy is a federally funded training facility for youth between the ages of 18 and 24. The program operates year round with approximately 200 trainees. The Academy is operated by ResCare, Inc. for the U.S. Department of Labor. SCOPE OF WORK: It is the sole responsibility of the selected vendor to acquire and provide all necessary proprietary diagnostic and programming equipment needed to support operations and service. Monthly examination of elevators, including oiling, and cleaning machinery, motor and controller, greasing or oiling bearings and guides, and making necessary minor adjustments. Make sure elevators are in the safest condition. We warranted repair or replace the following: Rollers, gibs, brake linings, slip-rings, limit switches, brushes, resistance and grids, contracts, shunts, springs, fuses, insulators, relays, braids, governor cable, Interpol, operator motors, operator belts, close limit, open limit, arc deflectors, carbons, coppers, guide rollers, replaceable shoe gibs and cleaning car tops and pits. Furnish lubricants of special grade compounded for elevator specifications. Comply with written violations from the local code enforcing agency and recommendations of insurance carrier on due notice of the owner for repairs covered. Respond promptly (within 2 hours) to emergency calls 24 hours a day, seven days a week at no charge. PERFORMANCE PERIOD: This agreement shall be in effect from the date of approval to March 31, 2015. Successive renewal of this contract may be made subsequent to annual solicitation of services following the first year. Additional work may be requested from time to time, which shall be authorized by written change to the resultant agreement. SAFETY AND HEALTH: All work shall be accomplished in a safe and efficient manner, with minimal disruption to the operation of the Academy. The subcontractor represents, by acceptance of any resultant agreement that they are in compliance with all regulations and standards flowing from the Occupational Safety and Health Act of 1970 (P.O. 91-596, 29 USC 651 et seq.) and further that in the event hazardous materials are involved, will comply with Federal Standards No. 310 dated July 1971 to the latest change, entitled "Symbols for Packages and Containers for Hazardous Chemicals and Materials." Vendor acknowledges that by acceptance of any resultant agreement, that all work and materials used in the performance of the contract duties meet federal, state, local and OSHA regulations. REQUIREMENTS: • Must be in compliance with all Local, State and Federal Guidelines and Regulations • Work to be accomplished in safe and efficient manner with minimal or no disruption or endangerment to trainees and/or staff. • Must provide a copy of current licensure and insurance • Must be registered on the SAM.gov Website • Must complete Vendor Packet. Which consists of the following items: • New Vendor Form. Fill out to entire form accordingly. DUNS SECTION IS A PRIORITY. • W-9 Form • Non-Conflict of Interest • Terms & Conditions and • Anti-Bribery Form - Sign Approval box. PROPOSAL SUBMISSION: Interested parties should respond to the above solicitation by the COB on October 22, 2014 to be eligible for award. Please reference this announcement number on your sealed envelope.
