Description

The Department of Veterans Affairs, Fayetteville Health Care Systems of the Ozarks (VHSO), intends to solicit proposal from contractors to provide Mobile Lithotripsy equipment and related technical personnel and support to and at the VHSO. The VA anticipates issuing a solicitation for this requirement on or about November 3, 2014, with a closing date for proposals on or about November 18, 2014. The solicitation will be available for download from www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments. Contract with Veterans Health Care System of the Ozarks (VHSO), Fayetteville, Arkansas shall be in accordance with a regularly scheduled day(s) per month to be mutually agreed upon between the parties (subject to increase by mutual agreement between VHSO and Contractor). The VHSO anticipates an average of four (4) veterans will be treated per regularly scheduled visit. The per-procedure pricing shall include single stone and multiple stone procedures per unilateral or bilateral episode. The equipment must be prepared to perform the first procedure of the day at 8:00 a.m. and continue to perform procedures until the VHSO's schedule for the day is completed. Contractor equipment must fit through elevator doors of 4 foot wide and 7 foot tall. Contracts will have a base term of one year, with options for four additional one-year terms exercisable at the discretion of the Contracting Officer. North American Industry Classification System (NAICS) Code is 621399 and size standard is $7.5 Million and the solicitation will be issued for 100% Small Business Set-aside. All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in SAM (System for Award Management) database (information on registration requirements can be obtained by the Internet at https://www.sam.gov and submit their complete Annual Representations and Certifications (ORCA) via online at https://orca.bpn.gov at proposal due date/time.
