- This combined Synopsis/Solicitation is for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are requested and a written solicitation will not be issued. The solicitation number is VA256-15-Q-0053 and is issued as a Request for Quotes (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-52. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 500 employees. This requirement is restricted to small businesses, and only qualified offerors may submit quotes. The solicitation will start on the date this solicitation is posted to FBO and will end on 24 October 2014 at 12 noon CDT. Destination shall be Shreveport, LA 71101. The VHA VISN 16 - 667 - Overton Brooks VA Medical Center requires the following items, Brand Name or Equal to the following - NEW EQUIPMENT ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. 0001, THYMATRON ® SYSTEM IV ECT INSTRUMENT - Made in the USA. CSA Approved. Includes: built-in 4-channel hard-copy monitor/printer (dual EEG, ECG and true EMG), front panel FlexDial programmer; 2 pads of thermal recording paper; 1 ECT Treatment cable; 1 EEC/ECG/EMG monitoring cable; 1 set monitoring leads (new clip style); 1 MouthGuard oral protector (large); 1 MouthGuard oral protector (small); 50 pair Thymapad ® disposable treatment electrodes; 200 count disposable EEG/ECG/EMG recording electrodes; 1 Drs. Abrams and Swartz' In-Service DVD; 2 ECT instruction manuals; 1 service manual; and 2 fuses. Also includes continuous heart rate printout, Krystal-Weiner EEG pattern analysis, patented auto EEG/EMG endpoint detection, the most efficient charge rate program 0.5mSec plus the 0.3 or.025 mSec pulse width program and the Genie IV Electronic Record software. Reveal problems in technique (e.g., recording electrode application) that are correctable on site or with user-replaceable parts (e.g., lead wires) Performs output checks Single button press instantly tells you if your ECT device is operating safely Patient Simulator mode that generates EEG, ECG, and EMG signals derived from real patients for testing up to 4 channels of your monitor/printer tracing display The good-quality seizure shows a high amplitude EEG followed by electrical silence at termination, with a pronounced tachycardia response and a high-amplitude EMG that terminates shortly before EEG termination.
