- Applicable Clauses from the FAR Request for Quote (RFQ) including a list of deliverables, and statement of work. This posting solicites proposals for Quarterly Health Assessments at the agency location in Washington, DC. The assessments will each be in three parts, with the first covering blood work, the second fitness testing and the third providing a printed report for use in counseling individual members (the report will be given to the member). Members who decline the counseling session will have the option of having the report mailed to them. In addition to this quarterly fitness assessment, an annual fitness fair to be held in the August/September is to take place at both locations (not simultaneously). Subjects should include but are not limited to Glaucoma testing, lung capacity, percentage of body fat, blood pressure, osteoporosis testing, and skin damage testing. Interested parties should read the attached Statement of Work and provide costs per individual for CLINs 001-004 and optional CLIN 007. Provide a price per seminar for CLIN 005. For CLIN 007 and optional CLIN 008 provide a total price. Please provide prices for the base year and four additional option years.
