Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 - streamlined procedures for commercial items, in conjunction with FAR Part 13 - Simplified Acquisition Procedures, specifically FAR Subpart 13.5 - Test Program for Certain Commercial Items; and as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-77, dated October 10, 2014. Solicitation VA262-14-Q-0037 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm Fixed Price award with a base year and up to four (4) option years. This procurement will be issued under a total small business set-aside. NAICS Code is 541690 and size standard is $15 million. The Government is seeking a Bi-annual Inspection, Certification, and Testing of Government-Owned Fume Hoods and/or Ventilation Devices, Biological Safety Cabinets (BSC), Laminar Flow Devices, Isolators, Biosafety Hazard Hoods, Sterile Workbenches, Radiation Safety Hoods, Buffer and/or Ante spaces, and Pharmacy Clean Rooms. Inspection, certification, and testing shall be in accordance with the following policies and recognized standards; and any other recognized standards within the industry norm: i.Federal Standard 209E, Class 100 ii.California Administrative Code. Title 8 iii.NSF/ANSI 49 iv.CDC/NIH Biosafety in Microbiological and Biomedical Laboratories, 4th Ed., May 1999 v.NIH Guidelines for Research Involving Recombinant DNA Molecules, January 2001 vi.OSHA Technical Manual - Controlling Occupational Exposure to Hazardous Drugs vii.American Society of Hospital Pharmacists Technical Bulletin on Handling Cytotoxic Drugs in Hospital viii.Food and Drug Administration ix.Joint Commission x.CAL/OSHA 5154 xi.International Organization for Standardization (ISO) Class 5, 7, and 8 Criteria xii.ISO 17025 and 1466-1 xiii.CAG-003 and CAG-009 xiv.IEST-RP-CC002 xv.United States Pharmacopeias (USP) 797 and 1116 Place of performances: (1) VA Long Beach Healthcare System, 5901 E. 7th St., Long Beach, CA 90822, (2) VA Loma Linda Healthcare System, 11201 Benton St., Loma Linda, CA 92357, (3) VA Greater Los Angeles Healthcare System - West LA, 11301 Wilshire Blvd, Los Angeles, CA 90073, and (4) VA Greater Los Angeles Healthcare System - Sepulveda, 16111 Plummer St., Sepulveda, CA 91343. See attached SF1449 Solicitation for statement of work (SOW), clauses and provisions applicable to the solicitation, other pertinent information. Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on a Acceptable/Unacceptable evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government. Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document (unless electronically completed on www.sam.gov ). As an alternative, Offeror(s) may use the Price/Cost Schedule of the attached SF 1449 for quote submission. Submit all question(s) to Tue Minh Tran at tue.tran@va.gov no later than October 31, 2014, 12:00 PM Pacific Standard Time (PST). Ensure to reference solicitation # VA262-15-Q-0037 within subject line of the email.
