FLOOR MAT RENTAL SERVICE FOR THE PHILADELPHIA VAMC AND AFFILIATED LOCATIONS. This is 100% small business set aside. The Philadelphia Veterans Affairs Medical Center (PVAMC), a division of the United States Government's Department of Veterans Affairs (VA), has a need for a qualified, technically capable vendor who can provide mat rental service for the VA Medical Center located at University and Woodland Ave., Philadelphia, PA 10104, the Community Liver Center (CLC) located at 3461 Civic Center Blvd., Philadelphia, PA 19104, the Philadelphia VA Medical Center/Dialysis Unit Community Based Outpatient Clinic (CBOC) located at 4219 Chestnut Street, Philadelphia, PA 19104, the Residential Rehabilitation Treatment Program "Snyder House" located at 1425 Snyder Avenue, Philadelphia, PA 19145, and the Camden Community Based Outpatient Clinic (CBOC) located at 300 Broadway, Suite 103, Camden, NJ 08104. The contract will include one base year only and will go into effect from date of award.
