Description: AMENDMENT 00002 - The purpose of this Amendment is to add a new Labor Category and correct items within the SIR/RFO. The SIR/RFO Closing date has also been extended to Monday, November 10, 2014 at 2:00 p.m. CST. AMENDMENT 00001 - The purpose of this Amendment is to provide answers to the questions submitted by potential offerors and to correct items within the SIR/RFO. The SIR/RFO Closing date has also been extended to Wednesday, November 5, 2014 at 2:00 CST Reference the Amendment 00001 and the two attachments (Questions & Answers, plus a revised Statement of Work) ------------ The Federal Aviation Administration (FAA), Mike Monroney Aeronautical Center (MMAC), Oklahoma City, Oklahoma, has a requirement for Civil Aerospace Medical Institute (CAMI ™s) Aerospace Medical Certification Division (AMCD) Administrative Services Contract. The FAA contemplates Single award, Indefinite Delivery/ Requirements type Labor-hour contract for Administrative Services. The resulting contract will have an expected Period of Performance (PoP) of three years with a base period and two (2) 1-year option periods. (This requirement was formerly posted as œMarket Survey Airman Medical Certification Division Support Services Contract for the Civil Aerospace Medical Institute # 15225 with a NAICS 541990, All other Professional, Scientific and Technical Services, with a Small Business Size Standard of $14 Million. After further discussions the NAICS has been updated to 541611) NO PHONE CALLS PLEASE. ALL QUESTIONS MUST BE IN WRITING AND EITHER MAILED, FAXED OR EMAILED TO THE CONTRACTING OFFICER. The North American Industry Classification System (NAICS) code for this procurement is 541611 œAdministrative Management and General Management Consulting Services All interested sources should first attempt to download the attached SIR document(s). If downloading is not possible, all requests for copies of the SIR document(s) must be made in writing to the attention of Monique Pelletier, Contracting Officer (AAQ-731), via e-mail at monique.pelletier@faa.gov or FAX number 405-954-9648. Proposals are due no later than 2:00 p.m. local time (Central Time Zone), October 31, 2014 and must be submitted to FAA, Customer Service Desk (AAQ-700), Physical Address: 6500 South MacArthur Boulevard, Oklahoma City, Oklahoma 73169. Mailing Address: P.O. Box 25082, Oklahoma City, Oklahoma 73125. Any and all questions regarding this solicitation must be received by the Contracting Officer via email by October 15, 2014 by 2:00 CST to allow adequate time for the Government to respond. After this date, no further questions shall be received. The proposal must be sealed and labeled as follows: SIR/RFO DTFAAC-14-R-00600. if all requested information is not furnished in the offeror's proposal, the offeror's proposal may be determined to be non-responsive and ineligible for consideration for contract award. Offerors submitting proposals by hand-delivery will need to consider allowing sufficient time to process through the security procedures in place at MMAC. Overnight delivery of responses to this SIR/RFO may also be impacted if not sent in sufficient time to allow for the special mail-handling procedures in place at MMAC. In addition, This SIR/RFO is not to be construed as a contract or a commitment of any kind. The Government shall not be liable for payment of any costs attributed to the preparation and submission of requested proposal documentation.
