Description

The Network Contracting Office (NCO) 12, Great Lakes Acquisition Center, has a requirement for Perfusionist services for the Clement J. Zablocki VA Medical Center (Milwaukee VAMC) in Milwaukee, Wisconsin. The purpose of this solicitation is for the contractor to provide qualified clinical personnel, as well as administrative and management support, necessary to perform Perfusion services for the Milwaukee VAMC in accordance with the Statement of Work (SOW), specifications, and terms and conditions listed within this Request for Quote (RFQ). The contract period of performance will be from the date of award (anticipated January 1, 2015) for one year with four (4) one-year option periods that may be exercised at the discretion of the Government. All qualified offerors/firms who believe they can provide the required services are encouraged to submit a quote. The small business size standard for North American Industry Classification System (NAICS) 621999 is $14M. Offerors are reminded to complete registration in the System for Award Management (SAM) system, including Offeror Representations and Certifications-Commercial Items. The electronic version of solicitation will be available on or about November 1, 2014. Date for receipt of quotes is November 20, 2014 at 1:00 PM Central Standard Time (CST). The electronic solicitation may be obtained FREE at the Federal Business Opportunities website: www.fedbizopps.gov. Any amendments to this solicitation will also be available at this address. Offerors are advised that they are responsible for obtaining any amendments which are to be submitted with your response.
