Description
HCV Genotyping reagents and supplies for Siemens AutoBlot 3000H VISN 1 Pathology and Laboratory Medicine Service (P&LMS) on behalf of the Molecular Diagnostics Laboratory at West Haven VA requires FDA approved reagents and ancillaries compatible with the Siemens AutoBlot 3000H Automated HCV Genotyping System. The Siemens AutoBlot 3000H Automated System is the analyzer currently being used to perform HCV Genotyping testing at the West Haven Molecular Diagnostic Lab, the reference lab for VISN 1 Labs. The purpose of this action is to establish a VISN contract with Siemens Healthcare Diagnostics for HCV Genotyping testing kits/supplies. This agreement is for a firm fixed contract price for reagent kits and consumables for a five (5) year period. No vehicle for reagent procurement for this analyzer is currently in place. This agreement would ensure availability, ease of procurement, and fixed pricing for this testing and ensure a continuance of this service to all veterans within VISN 1, VA New England Healthcare System. This procurement is for HCV Genotyping assay kits to be used on Siemens AutoBlot 3000H analyzer owned by VA Connecticut, West Haven Campus, P&LMS, Molecular Diagnostics Department. The Siemens AutoBlot 3000H analyzer is currently performing this testing in the VACT clinical laboratory located at the following VISN 1 facility: "VA Connecticut HCS, West Haven Campus, 950 Campbell Avenue, West Haven, CT 06516; POC: Leslie Zucker, Lab Admin. Officer, phone # 203-932-5711 ext. 4474. The Contractor is required to provide a continuously stocked inventory of assay kits, supplies and consumables required to properly perform testing on the equipment such that equipment operations are not interrupted. These items shall be of the highest quality, sensitivity, specificity and tested to assure precision and accuracy. Expiration date must be clearly marked on assay kits and reagents. Unexpected changes in methodology/technology shall be at the expense of the Contractor. Alert/Notification of any delays in shipment as well as any or all technical advisory/recalls/alerts, prior to or simultaneously with field alerts should be forwarded to the designated individuals, lab chiefs, and the VISN 1 program office. Special handling for emergency orders of supplies: In the event that the supplies are found to be defective and unsuitable for use with the contractor's equipment, or the contractor has failed to comply with the requirements for routine supply delivery, the contractor is required to deliver the supplies within 24 hours of receipt of a verbal order for emergency delivery. If either circumstance has occurred, the contractor shall deliver to the government site in the most expeditious manner possible without additional cost to the government, the necessary consumables in sufficient quantity as required to allow operation of the contractor's equipment for one week (under normal government test load volume). If additional requests for emergency supply delivery are required by the government, they shall be honored by the contractor until the arrival at the laboratory of the monthly standing order/routine supplies delivery. Test Menu: Refer to Attachment A for a list of desired tests and their estimated annual volumes. The Government estimates the volumes listed in Attachment A, but does not guarantee volumes as listed; they are estimates ONLY. Assay: HCV Genotyping Versant HCV Genotyping 2.0 Assay Kit (LIPA) - PN# 06714844 (40 strips per kit) Versant HCV Amplification 2.0 kit (LIPA) - PN# 06713570 (40 tests per kit) AutoBlot Strip Tray - PN# 1520480 (25 trays per box) The supplies and reagents shall be shipped and replenished in quantities as requested by the individual site Lab using delivery orders off the established agreement. The Molecular Diagnostics Lab will request the kits and supplies on an as-needed basis. The West Haven site will be invoiced for their own delivery orders. The contractor will provide direct delivery to the West Haven VA Lab FOB Destination. West Haven VA Lab will order using established procedures and referencing the purchase agreement pricing. They will be invoiced for their own orders. No shipping charges- FOB destination. The Contractor will notify within 24 hours, all participating locations and the NCL Program Manager of any recalled reagents. This must be accomplished by telephone and/or certified mail. The vendor will provide sufficient replacement items to resolve the problem when possible so no break in services is experienced. The contractor will provide a quarterly report to the Contracting Officer's Representative (COR) that indicates the volume of tests purchased by VISN 1 site. There will be no additional charges for any reports required as part of this agreement. Outdates on reagents must be at least 6 months.
