Description

Synopsis: Please note that this is NOT a request for proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, James J. Peter VA Medical Center is currently conducting a market survey for qualified firms capable of providing "preventive maintenance and all repairs, parts and labor for three Luminex equipment located at the James J. Peters VA Medical Center". The three Luminex equipment's; Luminex Analyzer LX10009336404, Luminex Platform LXY09356101, Luminex SD LXSD09349002 maintenance can be brand name or equal. The North American Industry Classification Systems (NAICS) is 541711. Responses to this notice must be submitting in writing (email or fax) and must be received not later than November 17, 2014 at 3:00PM. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 541711. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in Central Contractor Registry (CCR) (see internet site: http://www.ccr.gov). Peters VA Medical Center 130 West Kingsbridge Road Luminex Preventive Maintenance STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Project: Service Contract 2.Scope of Work: The contractor shall provide original equipment manufactured parts and software as well as service by trained technicians with experience enabling them to provide maintenance repairs as well as upgrades on the equipment listed below located at the James J. Peters VA Medical Center's MIRECC department. The contractor shall provide one preventive maintenance visit, the cost of replacement parts, labor and travel with a 48 hour response time. "Luminex Analyzer LX10009336404 "Luminex Platform LXY09356101 "Luminex SD LXSD09349002 The contractor shall provide preventive maintenance and all repairs, parts and labor for the Luminex equipment listed above located at the James J. Peters VA Medical Center. The contractor shall complete the work required under this SOW in 365 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. The work shall begin within 10 calendar days of award, unless otherwise specified. [Both] Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. 3.Background: The Luminex equipment listed is used for the VISN 3 MIRECC Neuropsychiatric research program and Brain Bank. This equipment is an integral part of the research program. Manufacture warranty requires maintenance of the Luminex equipment to be accomplished with original equipment manufactured parts, instruments and software. 4.Type of Contract: This is a full service Firm-Fixed-Price contract. 5.Place of Performance: The Veterans Health Administration of NY & NJ (VISN 3) James J.
