Description
DESCRIIPTION: Pest Control Services at the Palmdale ARTCC Group located in Palmdale, California. 1.LOCATION: Federal Aviation Administration Palmdale Group Attn: Mike Romero, Contracting Officer Representative 2555 E. Avenue P. Palmdale, CA 93550 B002. Performance Time: See PART I - SECTION F, clause 3.2.2.3-71 Starting, Performing, and Completing Work. B003. Solicitation Questions: All contractors proposing this project desiring an interpretation or clarification of the Specifications, drawings, contract terms and conditions, etc., must make the request by e-mail to Michelle Sites at michelle.sites@faa.gov. Alternately, you may fax your written questions to (858) 573-5594. Telephone questions will not be accepted. The requestor shall provide a company name, point-of-contact name, address and telephone number; as well as a return e-mail address or FAX number. The Contracting Officer (CO) is the only persons authorized to make clarifications, interpretations, or changes to this solicitation. QUESTIONS ARE DUE by: November 28, 2012 at:12:00 PM PACIFIC Time. B004. Site Visit: There is a mandatory site visit on November 26, 2014, at 10:30 AM PACIFIC Time. a. The meeting point address for the site visit. Federal Aviation Administration Palmdale Group 2555 E. Avenue P. Palmdale, CA 93550 POC: Luventhius haywood-Lee (661)265-8494 b. The FAA facility escort has been instructed not to answer any questions about the project during the site visit, please do not ask them any. All questions from the site visit must be submitted in writing according to the RFO s instructions in Section B, Clause B003 Solicitation Questions (above). The FAA assumes no responsibility for any conclusions or interpretations made by the contractor based on the information received by any means other than in writing from the Contracting Officer. The FAA does not assume responsibility for any understanding reached or representation made concerning conditions, which can affect the work, by any of its officers or agents before the execution of this contract, unless that understanding or representation is expressly stated in the contract. c. To attend the site visit, advance notice by e-mail is required by November 20, 2014 at 12:00 PM PACIFIC Time to the FAA POC identified below. You are required to provide your company representative s name. Primary POC: Luventhus Haywood-Lee(661) 265-8494 B005. Tiered Evaluation. A tiered evaluation of offers will be used in source selection for this contract, and offers from other-than small business concerns will only be considered after the determination that an insufficient number of offers from responsible small business concerns were received (see Part IV, Section M for details). B006. Small Business. Small businesses competing for this acquisition shall certify their status on the Business Declaration form inserted following Part IV, Section K. See Contract clauses 3.6.1-1, 3.6.1-8 and 3.6.1-12 in Part II, Section I. The applicable North American Industry Classification System (NAICS) code for this project is ESTIMATED PRICE RANGE FOR THIS PROJECT: $6,000 to $9,061.50 SERVICE CONTRACT ACT WAGE RATES APPLY - SEE SECTION J . OFFERS SHALL INCLUDE ALL APPLICABLE STATE AND LOCAL TAXES. SUBMIT OFFERS TO: SEE PART IV, SECTION L. PROPOSALS ARE DUE NO LATER THAN December 2, 2014 at 3:00 PM PACIFIC Time. THIS IS A REQUEST FOR OFFERS, THUS NO FORMAL BID OPENING WILL OCCUR AND RESULTS WILL NOT BE AVAILABLE BY TELEPHONE. DO NOT CALL FOR RESULTS. Scope of Project Furnish all necessary labor, material, equipment, transportation, and services to provide pest control at the Palmdale ARTCC. The Contractor shall: Treat for pest control but not limited to rats, mice and other rodents as well as mosquitoes, termites, cockroaches, spiders, bedbugs, ants, ticks, fleas, moths, bees and wasps. Inspect and treat exterior of all buildings, hardscape, trash enclosures and landscaped areas to control general pets (ants, spiders, roaches etc.). Service all rat and mice bait stations. Frequency of service Bi Monthly October, December, February, April,( Monthly) May, June, July, August, and September. (9 Services) This would include 6 call outs for emergencies. Details of service to be provided: 1)Inspection from top to bottom, entire area (working space3s and outside perimeter of facility). 2)Chemical Safe to human, make sure that MSDS sheets are submitted before use. 3)Trap Set traps, clean and removal of traps as frequent as necessary. 4)Removal of wasp/bee nest, rodent drops and dead pests. Submittals The contractor shall submit the following materials to the COR for his/her approval prior to using any chemical on Federal Aviation Administrative Property. Material Safety Data Sheets (MSDSs) for all materials to be used during the installation and incorporated into this project. Work Schedule All work shall be scheduled and performed at the designated times coordinated with the COR. All work shall be performed so that normal Air Traffic Control operation may continue with a minimum of disruption or interference. Security Requirements The Contractor shall comply with all security requirements of the Government. See page 17, Section I Contract Clauses 3.14-2 Contractor Personnel Suitability Requirements; 3.14-3 Foreign Nationals as Contractor Employees; and 3.14-4 Access to FAA Systems and Government-Issued Keys, Personal Identity Verification (PIV) cards, and Vehicle Decals. Access to Site Work performed at the site shall be by done under FAA escort at all times. The point of contact for access into the site is through the Mike Romero (661) 265-8417.
