- THIS REQUEST IS UTILIZING THE BRAND NAME/ BRAND SPECIFIC DESCRIPTION TO COMPLETE THIS OPEN MARKET PURCHASE ORDER. THIS IS DUE TO THERE ARE VARIOUS TYPES OF INSULIN PUMPS IN THE MARKET PLACE AND EACH PUMP HAS SPECIFIC SUPPLIES THAT ARE SPECIFICALLY MADE FOR THAT INSULIN PUMP AND ARE NOT INTERCHANGABLE. The Department of Veterans Affairs NCO15 CMOP Acquisition Department is seeking potential sources that have to capability to fulfill the complete requirement of the ANIMUS Insulin Pump supplies listed below. This request is for the Charleston SC CMOP at the following location: Department of Veterans Affairs, VA Charleston CMOP 3725 Rivers Ave, Suite 2 North Charleston, SC 29405-7038. The Product Code used for this procurement request is 6515; with the NAICS code of 339112. THIS POTENTIAL PURCHASE ORDER HAS A VERY STRENGENT DELIVERY LEAD TIME DUE TO THIS IS AN EMERGENCY REQUEST. DUE TO THIS REQUIREMENT, THE DELIVERY LEAD TIME MUST BE AT STATION NO LATER THAN 10 DAYS ARO. Full Delivery required NLT 10 days ARO, no partials accepted without prior approval. All responsible sources wishing to submit an Offer for this potential award must make sure that their offers and all information required is submitted. All offers shall be considered by this agency if received by the closing date and time of this RFQ. The following information MUST BE INCLUDED WITH ALL OFFERS: "Verification that this request will be completed as a One-Time Shipment without any interruptions. This Solicitation RFQ package# VA255-15-Q-0144 shall be posted thru FedbizOpps on or around November 14, 2014 and will close no later than 1630 (4:30) CST on November 17, 2014.
