Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-15-Q-0044 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-77. The requirement is for Brand Name or Equal for ThermaSure Model 109 Double Door Dryer and accessories with extended warranty. This is a small business set-aside and the associated NAICS code is 334510 and small business size standard is 500 employees. Delivery location is Cleveland VAMC in Cleveland, Ohio and shall be FOB destination. Delivery and installation required within 30 days of contract award. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-77. Solicitation closes on November 21, 2014 at 5:00pm EST PRODUCT DESCRIPTION/PRICE SCHEDULE Louis Stokes Veterans Affairs Medical Center in Cleveland, Ohio located at 10701 East Blvd, Cleveland, Ohio 44106, is looking to procure Brand Name or Equal for ThermaSure Model 109 Double Door Dryer and accessories with extended warranty. The ThermaSure and accessories will be placed in the Sterilization Processing Service (SPS) Department. Cenorin is the manufacturer for the ThermaSure and accessories. See the below Price Schedule for part/model numbers for all items. This will be a Firm-Fixed-Price Purchase Order. All brand name sellers certify that they are an Authorized Channel or Reseller as of the date of the submission of its quote, and that they have the authorization required to support the product sale, product pricing, and product warranties in accordance with applicable manufacturer authorization requirements.
