NOTICE TYPE: Request for Proposal FY 15 CEILING LIFT PREVENTATIVE MAINTENANCE INSPECTIONS (PMI) Lebanon VA Medical Centre, VISN 4 - Lebanon, PA Campus This RFP notice is for the purpose of an eventual award. Please provide information and description of services available. Results from this Request for Proposal will help identify an offeror for an eventual award. The Lebanon VAMC, located @ 1700 South Lincoln Avenue in Lebanon, PA requires full service maintenance support for hospital ceiling lifts as prescribed within the statement of work listed below. Responses to this notice must be submitted in writing (via E-mail) referencing the requirement & RFP # and must be received not later than 1 December 2014 at 4:00PM. Email: kelly.byers@va.gov. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, description of item(s) available, DUNS number, address, point of contact and social-economic status verification. To be eligible for consideration, offerors will have been engaged in maintaining/servicing the type of equipment listed for a period of no less than two (2) years and be capable of presenting certification of such with verifiable references. No allowance will be made for a company not having qualified and certified personnel available to respond with prompt and satisfactory service. Submissions in response to this announcement and to be eligible for consideration for this effort must submit a record of their ability to maintain and service the type of equipment listed in the statement of work listed below. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business) for an eventual award. STATEMENT OF WORK CEILING LIFT PREVENTATIVE MAINTENANCE INSPECTION (PMI) A.GENERAL INFORMATION 1. Title of Project: FY15 PMI Ceiling Lifts 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall provide all labor, materials, tools and equipment necessary to provide preventative maintenance and repair to the ceiling lifts. 3. Background: Ceiling lifts are used for patient care and they must be inspected and maintained to ensure they are safe for patient use. 4. Performance Period: The Government is requesting one base period of one year and four option periods of one year. Base Period: 1 Oct 2014 thru 30 Sep 2015 Option Period 1: 1 Oct 2015 thru 30 Sep 2016 Option Period 2:1 Oct 2016 thru 30 Sep 2017 Option Period 3:1 Oct 2017 thru 30 Sep 2018 Option Period 4:1 Oct 2018 thru 30 Sep 2019 B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS Scope of Work: IN ACCORDANCE WITH THE FOLLOWING SPECIFICATIONS: "Quantity of 205 Pneumatic System checks, for the hand control, airline tubing, and grommet connectors. IAW SOW Para A "Quantity of 205 Electrical System Checks, for the batteries, charger and connections, wire harnesses, up & down limit switch, LED indicator & LCD display, and electric On/Off, emergency lowering & emergency shutoff. IAW SOW Para A "Quantity of 205 Mechanical System checks, for the carry bar, lifting strap, trolley wheels & chain drives, and motors and gears. IAW SOW Para A "Requirement of two (2) report binders with Room Number locations, PMI Numbers, and ELI Numbers. IAW SOW Para A "Include Inspection Stickers that are completed and placed on equipment that has been inspected. IAW SOW Para A "Perform preventive maintenance on our ceiling lifts at the Lebanon VA Medical Center. Two PMs on each ceiling lift will occur every year. The PMs will be conducted on the following timeframe: 1st PM will occur between the months of October and December 2014, and must be completed by the end of December. The 2nd PM will occur no earlier than 6 months from the 1st PM, and no longer than 7 months after the 1st PM. All PMs will be documented and be given to the Lebanon VA Medical Center Engineering Office in Building 99. Requirement of two (2) report binders with Room Number locations, PMI Numbers and ELI Numbers. Include Inspection Stickers that are completed and placed on equipment that has been inspected. "In the event that a battery is found to be dead or unserviceable, contractor will be responsible for making necessary corrections.
