Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-77. This combined synopsis/solicitation is set-aside 100% for small business. North American Industrial Classification Standard 314110 applies to this solicitation; business size standard is 500 employees. The following is needed: This requirement is to cover two separate areas of flooring with carpet with option for installation. Area (1) needs approximately 460 (419 actual square yards plus 10% overage) and the second area, Area (2) needs approximately 350 (319 actual square yards plus 10% overage) square yards. Request separate quotes for each area. Additionally each quote should list product cost separate from installation cost. Seeking carpet that resists stains and withstands wear for high traffic area. Ease of installation and replacement is a factor. Suggested product includes Interface Transformation, Tile Size: 20 x 20 inch, Thickness: 0.28 inch Samples will be accepted. Adhesives are to be compatible with materials being adhered. If installation is elected, desired installation date is no later than 12 January 2015. Installation shall be completed in accordance with manufacture installation requirements. Pictures of Area (2) are included. Standard commercial warranty should be included and the specifics detailed. Vendor must provide a list of all items to be provided, the quantity and country of origin. For any additional features, please price as options. ****Multiple solutions to the requirement should be offered as alternative quotes and will be evaluated as separate offers. Multiple solutions are encouraged.**** Innovative solutions are encouraged and will be evaluated in accordance with the intent of this requirement. Provision 52.212-2 Evaluation-Commercial Items is amended to read: ''Evaluation Factors and Sub Factors: -Price -Past Performance -Technical Capability Subfactors -Performance indicators and feasibility -Delivery schedule -Conformity to desired features All factors are of equal weight. All subfactors are of equal weight. A SITE VISIT is optional and scheduled for 2 December 2014 at 10:00 am. Interested attendees MUST RETURN completed FW form 66 (attached) to 148FW.MSC@ang.af.mil NLT 25 November 2014. All questions and quotes must be directed to SMSgt Tana Johnson via email at 148FW.MSC@ang.af.mil. Questions asked within 3 days of solicitation close will be answered at the discretion of the Contracting Officer. Please make quotes good for 30 days if possible. Emailed quotes are preferred. Quotes must be received by no later than 1:00 PM CST, 08 December 2014. Late quotes may be rejected at the Contracting Officer's discretion. ****Quotes may be comparatively evaluated in compliance with FAR 13.106-2 (b) (3).****'' Multiple quotes are encouraged with varying items and optional accessories. Include past performance information: other military, commercial, and civilian companies with whom your products have been sold to. Include country of manufacture for all items.
