- Service Locations Relevant Company Information Sources Sought Notice for information on capability and availability of potential contractors interested in proposing on a Blanket Purchase Agreement (BPA) for Laboratory Services for the U.S. Army Corps of Engineers, South Atlantic Division (SAD), covering the states of Alabama, Florida, Georgia, Mississippi, North Carolina, South Carolina, Tennessee, Virginia and occasionally network locations throughout continental U.S. The proposed agreement duration will be for an undetermined period. This is a SOURCES SOUGHT SYNOPSIS. The purpose of this sources sought synopsis is to determine the availability of qualified contractors to perform these services to include Certified Small Businesses, HUBZone Certified Small Business Concerns, Section 8(a) contractors, Service Disabled Veteran owned Small Businesses, Small Disadvantaged Businesses, Women Owned Small Businesses and Large Business Concerns. The primary North American Industrial Classification System (NAICS) Code applicable to this requirement is 621511. The small business standard for which is a concern, including its affiliates whose average annual gross revenues does not exceed $32.5 M for the past three years. Responses to this Sources Sought Synopsis may be used by the government to establish BPAs. Laboratory services under these BPAs will be provided to federal employees of participating agencies in accordance with the SAD Regional Occupational Health Center (ROHC), Public Health Regulations, and other directives/protocols. The Regional Occupational Health Center is located in the Federal Building, 109 St. Joseph Street, Mobile, AL. The ROHC will provide direction for laboratory services within the Region. Responses are requested with the following information, which shall not exceed a total of five (5) pages (not to include Relevant Company Information): 1. Offerors' name, address, points of contact with telephone numbers and e-mail addresses. 2. Confirmation of offerors' active System for Award (SAM) registration and valid Tax Identification Number. 3. Business size/classification, to include any designations as Certified Small Business, HUBZone Certified Small Business Concern, Section 8(a) contractor, Service Disabled Veteran owned Small Business, Small Disadvantaged Business, Women Owned Small Business or Large Business Concern, shall be indicated on first page of submission. 4. A minimum of two (2) but no more than five (5) projects completed within the last five (5) years from the date of this notice which demonstrate experience with laboratory services by fully completing the Relevant Company Information form (Attachment 1). 5. Documentation should be submitted with the following considerations: a. Describe how your laboratory could ensure proper handling in obtaining specimens from the point of collections, typically located in and/or around the service areas listed in Enclosure 1, for laboratory analysis. Also, in Enclosure 1, mark with an quote mark X quote mark the service areas your laboratory is able to service. b. Discuss your capability of providing daily test results for employee/patient via a uni-direction interface, which allows laboratory to electronically transmit results into the ROHC's Occupational Health software Medgate. The results need to be based on HL7 v2.2 and v2.3 standards. c. Describe your ability to assign an accounting code for each of the five (5) regions of the ROHC (Charleston, SC; Jacksonville, FL; Mobile, AL; Savannah, GA; Wilmington, NC) to an individual employee's lab invoicing. d. Describe your network of U.S collection sites. e. Confirm your capability of combining the lab panels that we are requesting as follows: A. Panel 1 (Standard Lab Panel) 1. Chemistry profile to include Glucose, Uric Acid, BUN; Creatinine, Serum; BUN/Creatinine ratio, Sodium, Potassium, Chloride, Calcium, Phosphorus; Protein, Serum; Osmolality (calculated); Albumin, Globulin, A/G ratio; Bilirubin, total; Alkaline Phosphatase, LDH, SGOT, SGPT, GGT, and T4 & TSH. 2. Lipids to include Cholesterol, total; Triglyceride, HDL, VLDL, LDL ; T. Chol/HDL ratio and Estimated Cardiac Risk. 3. Complete Blood Count to include WBC, RBC, Hemoglobin, Hematocrit, MCV, MCH, MCHC, Platelets, Polys, Lymphs, Monocytes, Eos, Baso, and Differential. B. Panel 1A - Standard lab panel with the addition of a routine urinalysis with microscopic as indicated. C. Panel 1B - Same as panel 1A with the addition of Hemoglobin A1C. D. Panel 2 - Standard panel 1A with addition of PSA. This panel is usually recommended for men above the age of 50 years and for men 45 years or older with a high risk for prostate cancer. E. Panel 2A - Panel 2 with addition of hemoglobin A1C for diabetics. Responses should be submitted via email to Sophia Chin, Contract Specialist, at sophia.m.chin@usace.army.mil. ATTN: Plans Room, Contracting Division, Sophia Chin/CT-S. Submittals are due on Friday 5 December 2014, at 4:00 p.m. CST. No responses will be accepted by fax. Submittals will not be returned. Telephonic responses will not be honored. Note 1: This is not a Request for Proposal, only a Request for Information. No contract award will result from this Sources Sought; although agreements may become available. Note 2: This sources sought synopsis is not to be construed as a commitment by the Government, nor will the Government pay for the information solicited.
