Description

THIS IS A SOURCES SOUGHT NOTICE ONLY NOT A REQUEST FOR PROPOSAL (RFP). The purpose of this sources sought is to gain knowledge of potential qualified sources and their size classification (hubzone, 8(a), small, small disadvantage, small veteran owned business or service disabled veteran owned small business) relative to NACIS 62111 (size standard of $11 Million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps.gov website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Northern California Health Care System is seeking sources for Health Care Resources in the form of Gastroenterology and Colonoscopy Services. Gastroenterology services are to be provided at the contractor's place of business. The contractor shall furnish all labor, tools, equipment, materials, supplies, management, supervision, transportation, teaching, consultations and reports as required for the execution and completion of work necessary. The VA Northern California Health Care System (VANCHCS) requires access to Gastroenterology Services in two specific geographical areas. The restrictions will enable VANCHCS to better serve patient care needs by providing easy access and options. The VANCHCS prefers the Contractor's facility to be located within a fifteen (15) miles radius of 425 Plumas Street, Yuba City, CA 95991 (Yuba City OPC); or 10535 Hospital Way, Mather, CA 95655 (Sacramento VA Medical Center). If you are interested, and are capable of providing the required services please provide the requested information as indicated below. The intended contract period will be for a base year and two, one year option periods, which is to begin on or about January 01, 2015. Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: (1) Is your business small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified 'Hubzone' firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small Business, (7) Is your firm a Veteran Owned Small Business? Small businesses considering participation in this potential solicitation process under any Small Business Set-aside procedure must do so in a manner consistent with the requirements of the Small Business Act. Responses to this notice shall be e-mailed only to the attention of Nandini Johnson, Nandini.johnson@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 12:00 p.m. PST December 5, 2014. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
