- This is a combined synopsis / solicitation for commercial items prepared in accordance with (IAW) the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number, W81K0-15-T-0022, is therefore issued as a Request for Quote(s) (RFQ). This solicitation document incorporates provisions and clauses IAW the Federal Acquisition Regulation Circular 2005-74 and DPN 20140528. FAR Clause 52.211-6, Brand Name or Equal applies to this acquisition. This solicitation is for Radiopharmaceutical Isotopes for use at the San Antonio Military Medical Center, Ft. Sam Houston, TX 78234-6200. The anticipated period of performance is, 01 January 2015 - 30 September 2015. The associated North American Industrial Classification System (NAICS) code is, 325412, with a size standard of 750 employees. Offers are due by 09 Dec 2014, 12:00 PM, Central Standard Time, via email to the point of contact, Elisa M. Mendez, elisa.m.mendez@us.army.mil. Questions shall be submitted via e-mail, no later than, 05 Dec 2014. This procurement is being conducted on an un-restricted basis. Eligible business concerns may submit a quote, at which time will be considered. This is a quote mark brand name or equal quote mark solicitation. The brand name description of the radiopharmaceutical isotopes product is, Petnet Solutions Inc. Description of each item is listed below. The final unit prices established, resulting from this solicitation, when ordering as needed, will be applicable to any order placed under this contract. The Government will only pay for actual quantities ordered and accepted, regardless of the quantities established on the contract. Therefore, the offeror is hereby informed that the final unit prices are not subject to adjustment on the basis of a difference between ordered amount and actual quantities.
