Description

The VA Lebanon Medical Center intends to negotiate on a Sole Source basis with Beacon Point Associates, LLC, 21011 Johnson Street, Suite #109, Pembroke Pines, FL, 33029, to award a Contract to them under the Authority at Public Law 109-461 as codified at 38 U.S.C. 8127 (b), to furnish a Synthes Surgical Screw Extraction Set (Model #01.240.001), along with three additional Synthes surgical screw extraction items, as a turnkey requirement, in accordance with the following Statement of Need. Estimated Amount: $29,995.50. Screw Extraction Set Statement of Need Background: The Lebanon VA Medical Center (LVAMC) has a requirement for a screw extraction set. The screw extraction set will be used to remove screws that are damaged during surgery. In an effort to avoid delays and/or extensions of patient care, the acquisition of the screw extraction set will eliminate the practice of calling the vendor to deliver supplies during urgent procedures. Scope of Work: "Delivery of required screw extraction set within 30 days of contract award. "All instruments will be brand new.
