Description
Security Notice to Prospective Offerors Business Declaration Past Performance History The FAA is seeking competent and qualified Facilities Maintenance contractors to perform Janitorial services at Airport Traffic Control Tower (ATCT) Cedar Rapids, Iowa. The facility is located at 9455 Shepard Court SW Cedar Rapids, IA 52404-8954. NOTE: Requirements to be accomplished between the hours of 5:00 p.m. and 10:00 p.m. unless otherwise instructed. Below is a sample list of janitorial services to be provided at the above location. Below is a sample list of janitorial services to be provided at the above locations. 1. Empty all wastebaskets and replace plastic liners daily. 2. Vacuum all carpeted areas & remove spots daily. 3. Sweep all uncarpeted areas; damp mop and spot clean floors; 4. Light dust furnishings including banisters and railings daily. 5. Clean and sanitize restroom toilets, sinks, stalls and urinals daily. 6. Provide and replenish paper towels, dishwashing detergent, and hand soap in the restroom, breakroom/kitchen areas daily. 7. Clean all Partition Glass and Door Glass daily. 8. Clean and remove all foreign substances from floors, spot wax and buff weekly. 9. Dust surfaces of all offices furniture and fixtures weekly. 10. Sweep and wash all stairs and landings. 11. Wash Waste Receptacles weekly. 12. Remove all stains, smudges, marks, and stains monthly. 13. Wax and buff all non-carpeted floors monthly. 14. Vacuum all upholstered furniture monthly. 15. Wash All Bathroom Walls Quarterly. 16. Steam clean all carpeted floors Semi-Annually. 17. Strip and re-wax all non-carpeted floors Semi-Annually. 18. Wash all windows, Venetian Blinds, and Walls Semi-Annually. 19. Wash windows on the outside only annually. This list a small sampling of janitorial services to be provided and is NOT all-encompassing. The contractor will be required to provide all labor, materials, supplies, and equipment (except Government furnished) to perform this work in accordance with the statement of work provided with the SIR and with all local, state, and federal laws. This is NOT a Screening Information Request (SIR) or Request for Proposal (RFP). Work will be done on an operational facility and the contractor shall comply with all procedures, rules and regulations of the facility authorities including limitations on equipment, allowable work hours, and other issues to ensure uninterrupted operations. All work shall be in accordance with the plans and specifications. 1. This project is set-aside for SEDB/8(a) concerns to all Facilities Maintenance Contractors. 2. The North American Industry Classification System (NAICS) is 561720, Janitorial Services. The small business size standard is $18.0 million. 3. The project magnitude is between $150,000 and $250,000. 4. A mandatory site visit will be required. The date, time, location, and point of contact will be provided with the solicitation package. 5. The contractor receiving award is expected to perform at least 25% of the work utilizing their own employees. 6. Contractors must have an active registration in System Award Management (SAM) before award can be made. Contractors can register at www.sam.gov/. AN OFFEROR MUST MEET THE FOLLOWING GO/NO-GO CRITERIA IN ORDER TO RECEIVE A SOLICITATION PACKAGE: a. Using the Past Experience/ History form (Attachment #1), provide a listing of at least 3 past or current Facilities Maintenance contracts/projects your firm has performed in the same size ($150,000 and $250,000) and scope of this project. b. Contracts/projects must be either in process or within the last 5 years. Scope is described above. Similar work must include FAA Centers, Towers, or other life-critical facilities. Offeror shall identify the contracting agency, (FAA, Other Federal, State, local governments, and private). You must provide the contract number or project identifier, which includes a point of contact and phone number. The information provided will also be used as part of the responsibility determination. c. Complete and return the Business Declaration form (Attachment #2). d. 8(a) Certified (attach Certification letter). FAILURE TO SUBMIT ALL REQUIRED INFORMATION WILL DEEM THE REQUEST FOR SOLICITATION AS NONRESPONSIVE.
