- Terms & Conditions Vendor forms RFQ #2014HJCC-TMAF. Contains scope of work. To All Interested Contractors: This is a subcontracting opportunity with Management & Training Corporation (MTC), operator of the Hawaii Job Corps Center (HJCC) for the U.S. Department of Labor. MTC is requesting bids for plank flooring. Please provide an itemized price quote for the following good(s): 1)TrafficMASTER Allure Plank Flooring (Mfg Model #7113) a. 6" W x 36" L x 3.8 mm thickness, Bamboo Dark color, resilient vinyl, 24 sq ft/cs b. Quantity Requested: 100 cases Offer must be for specific item listed, alternatives and/or substitutes will not be accepted. Offer must be FOB destination and must be delivered in full as agreed upon by HJCC. Failure to do so will be considered a breach of contract. Price quote is to be inclusive of any taxes, fees, and delivery costs. Goods are to be delivered to: Hawaii Job Corps Center Attn: Receiving Dept 41-467 Hihimanu Street Waimanalo, HI 96795 BIDS MUST BE RECEIVED NO LATER THAN 4:00 PM (HAWAII ST) ON DECEMBER 19, 2014 VIA EMAIL (PREFERRED) OR DELIVERED TO THE HAWAII JOB CORPS CENTER. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final. All bidders are subject to MTC's terms and conditions. Bid Submission: A complete bid submission shall include: - Pricing on vendor's letterhead - Signed Acceptance of Terms & Conditions form - Completed Self-Certification form - W-9 MTC Terms and Conditions: Management & Training Corporation (MTC) Terms and Conditions are included with this RFQ, and can be found at www.mtctrains.com; a copy is also provided with this RFQ. It is the Contractor's responsibility to verify MTC's current Terms and Conditions at the time services and/or goods are provided. North American Industry Classification System (NAICS): Bidder must self-certify under the following NAICS code (NAICS codes can be found at http://www.census.gov/eos/www/naics/): 444130 (Hardware Stores) or 423310 (Lumber, Plywood, Millwork, and Wood Panel Merchant Wholesalers) Federal Funding and Accountability Act of 2006 (FFATA): For organizations, projects with a value of $25,000 or more (and any modifications to a project that change previously reported data) may be subject to the provisions of FFATA, per FAR 52.204-10 Reporting Executive Compensation and First-Tier Subcontract Awards. Reported information will be made public via the FFATA Sub-award Reporting System (FSRS.gov). For applicable subcontracts, this information must be provided to (MTC/Hawaii Job Corps) prior to the final award of the subcontract using the enclosed FAR 52.204-10 First-Tier Subcontractor Information form. The vendor must acquire a DUNS number to fulfill this requirement.
