- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SUBMITTALS ARE DUE 5 JANUARY 2015, 2:00 PM LOCAL TIME The contractor is to provide janitorial services for the Lake Michigan Area Office Wisconsin O&M Branch. Janitorial services shall be provided two times per week on Monday and Thursday (excluding holidays) starting no earlier than 8 a.m. and being completed no later than 12 noon. If a holiday falls on a day when the office is scheduled to be cleaned, the contractor shall come another day during that work week, but this schedule must be coordinated with the on-site government staff. The contractor shall provide all cleaning supplies and equipment including plastic bags for waste and recycling containers. Paper supplies (paper towels, toilet tissue) and soap will be supplied. The office area to be cleaned is approximately 5,400 square feet. The carpeted area is approximately 1,500 square feet. There are two bathrooms in the facility, with 1 sink and 1 toilet each. The office building is located at 2619 East Capital Drive, Appleton, WI 54911. 1. Work shall be performed at each cleaning, except as stated, in building: a. Vacuum all carpeted areas. b. Sweep non-carpeted floors c. Mop non-carpeted floors d. Dust desks (without disturbing paperwork), tables, cabinets, window sills, moldings twice a week. e. Empty garbage and recycling containers f. Clean restroom fixtures and water bubbler g. Wash inside of exterior windows and clean window treatments, minimum once per quarter year h. Wash all accessible sides of interior windows, once per quarter year i. Clean all accessible sides of glass doors j. Clean and wash lunchroom table and lunchroom sink k. Clean conference room tables, minimum once per week 2. The contractor is to provide all Material Safety Data Sheets (MSDS) for any chemical that is considered hazardous that they bring onto U.S. Government property per 29 CFR Part 1910.1200 (g). The MSDS shall be hard copy and be located at a central location where all employees may have access to them in the event of an emergency. The contractor will also be provided the opportunity to review all MSDS for government owned chemicals. 3. All chemicals which are brought and/or stored on U.S. Government property need to be properly labeled at all times, including the identity of the chemical and the appropriate hazard warnings. Proper labeling requirements are listed in 29 CFR Part 1910.1200 (f) 4. Contractor is responsible for establishing and maintaining an adequate quality control system to inspect and ensure that all work is satisfactorily performed in accordance with this contract. 5. The government will use the attached quality assurance surveillance plan, to monitor and evaluate the progress of the contractor.
