Description
This is a Pre-Solicitation Notice to inform potential vendors that the Government intends to Post an Official Solicitation at a future date, that qualified vendors may submit quotes for the Service for a base year with four option years. NOTE: This is set-aside for Certified VetBiz.gov SDVOSB vendors only. STATEMENT OF WORK Summary: The Contractor shall provide all supplies, materials, equipment, labor, supervision, management and transportation necessary to ensure that indoor plant maintenance services are performed at the Department of Veterans Affairs Health Care System, Portland Division, 3710 SW US Veterans Hospital Road, Portland, Oregon and the Department of Veterans Affairs Health Care System, Vancouver Division at 1601 E. 4th Plain Blvd, Vancouver, Washington in a manner that will maintain a healthy, pest and disease free, thriving, attractive and professional appearance of indoor plants and landscapes. Scope of Work: a)A minimum of one visit per week for inspection of plant needs and to provide periodic watering or misting, frequency to be determined by plant species, location (exposure to sunlight) and any other environmental factors. b)Maintain a healthy and vigorous plant appearance c)Remove dead, damaged or unsightly foliage d)Dust foliage, certain species may need regular spraying e)Adjust or change dirt cover (moss, rocks, etc) to maintain a uniform, healthy appearance f)Clean outside of pots/containers of soil and debris required g)Follow special plant requirements or instructions depending on environmental and species specific conditions h)Prune or trim plants as needed on species to encourage growth and healthy appearance i)Rotate plant location based on species light requirements or as advised by the contracting officer or POC. j)Apply fertilizer or other nutrients as required and adjust accordingly for flowing species. k)Inspect plants weekly to determine presence of any pests or disease and treat plants accordingly to ensure control of infestation or disease after consultation with POC. l)Repot plants for optimum growth and appearance m)Keep comprehensive inventory list of all plants and their specific locations. New plants, if and when required will be added to inventory list and updated inventory list will be provided to the POC within one month n)Submit total plant inventory list, complete with plant species and location once a year to the POC. New Plants, Plant Replacement and Special Event Plant Services: Replacement plants, when required will be handled separately by purchase order or government purchase card after consultation with POC. Upon notification by the contracting officer, the contractor shall perform special event plant services as required. Special event services will be priced separately from routine services, but may consist of all or some of the basic services within this contract. These services will be handled separately by government purchase card or purchase order. Disposal of Hazardous Wastes: The Contractor shall dispose of any excess pesticide containers or any other materials contaminated by pesticides at any location on the medical facilities premises except as specified by the POC. The Contractor will be required to furnish the POC, prior to initial application of chemicals, i.e. pesticides, fungicides or fertilizers, the trade names (if applicable), and chemical names of the chemicals used and a label showing the contents, the strength of the chemical, as applied, and the antidote thereto. Contractor will furnish the same information any time he changes chemicals or products used in the performance of this contract. This information is required by the Medical Service for emergency treatment in the event of ingestion of or contact with the material by humans. All chemicals used in the plant management process must have a Material Safety data Sheet (MSDS) provided to the POC prior to its use in the VA Health Care Systems, Portland and Vancouver facilities. Personnel Screening Compliance 1. The Contractor's records/documents must show that they appropriately screen individuals requiring access to the Medical Center's information and information systems prior to authorizing access. 2. The Contractor must maintain records or documents that show that the personnel screening control responsibility has been assigned and specific actions taken to ensure the implementation of this control. 3. The Contractor documents/records must show that they consistently conduct personnel screening for positions within their working unit on an ongoing basis. 4. When anomalies or problems are encountered by the Contractor in the implementation of the personnel screening control, these are documented and the resulting information is used to improve the control. Investigative Process for Contract Personnel: a. For suitability and security eligibility determinations within VA, contract personnel will be subject to the same investigative requirements as those for regular VA appointees and employees. When appropriate, exemptions may be applied as described in VA Directive 0710, paragraph 2c, Exemptions. The Security and Investigations Center initiates and adjudicates background investigations of contract personnel for Low, Moderate, and High Risk position designations. Non-citizen contract personnel appointed to Low Risk or Non-sensitive positions will be subject to a NACLC investigation, to be initiated by the Security and Investigations Center within 14 calendar days of appointment. b. Contract personnel may be provided brief or one-time access to non-national security VA information in the performance of their contract requirements without requiring a background screening or investigation. All such contract personnel will be escorted or overseen by a suitable VA employee designated by the facility or organization's ISO. Invoices: All payments shall be made in arrears upon receipt of a proper invoice. Invoices will provide the following minimum information: "Contract Number "Date of Service "Itemized Charges "Authorizing Official Location of Services: Vancouver Campus: VA Health Care System 1601 E. 4th Plain Blvd Vancouver, Washington 98661 Portland VA Health Care System 3710 S.W. US Veterans Hospital Road Portland, Oregon 97239 Recognized Holidays: The contractor is not responsible to provide services on the following days: New Years Day, Martin Luther King Day, President's Day, Memorial Day, Fourth of July, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas. Cellular Phones: The use of cellular and two-way radios is prohibited in all Portland and Vancouver VA Health Care System buildings. The equipment shall be turned off upon entering either of those buildings. They have been approved for use on the grounds of the facility. GEMS: The Portland VA Health Care System has a Green Environmental Management System (GEMS). The Medical Center is committed to protecting and improving the environment. Our goals are: - Taking a leadership role in environmental stewardship by providing a clean and safe environment in our community - Conserving natural resources and supporting their sustainability by upgrading our environmental systems - Reducing the use of hazardous products and the generation of wastes - Purchasing renewable, reusable, recyclable and recycled products - Working to constantly improve the immediate and long term environmental impacts of the products, services and processes used by the PVAMC Medical Center. The VA Health Care System Contractor must follow the spirit of GEMS and recycle all applicable material. The Contractor will follow the GEMS guidelines provided by the Portland VA Health Care System, GEMS coordinator.
