- Combined Synopsis-RFQ for AFMS HRO Symposium and Training Attachment 2 - Pricing Schedule Attachment 1 - Requirements Document The Request for Quotation number is FA8052-15-T-0008, AFICA-AFMS High Reliability Organization (HRO) Symposium and Training in San Antonio, Texas, from 11 - 13 Feb 2015 for approximately 50 attendees. Check-in will be on 10 Feb 2015 for approximately 30 attendees and 20 attendees will be checking in on 11 Feb 2015. Provisions and clauses in effect through Federal Acquisition Circular 2005-54 are incorporated. This requirement is 100% set-aside for Small Businesses under NAICS 721110, with a Size Standard of $30 Million (FAR 19.502-2(a)). All qualified small businesses under this NAICS code are encouraged to submit quotes. The Air Force Installation Contracting Agency (AFICA), 773rd Enterprise Sourcing Squadron (773 ESS/PK) has a requirement for hotel rooms and conference center facility to host the AFMS HRO Symposium and Training in downtown San Antonio, Texas, for the requirements described in the attached Requirements Document dated 30 Dec 2014 (Attachment 1). The Air Force Installation Contracting Agency (AFICA), 773rd Enterprise Sourcing Squadron (773 ESS/PK) is requesting a cost estimate and availability for hotel/motel rooms and conference center facility to host the AFMS HRO Symposium and Training in downtown San Antonio, Texas, from 11 - 13 Feb 2015 for approximately 50 attendees in accordance with the attached Requirements Document (Attachment 1). The resultant contract is not intended to cover the price of hotel rooms occupied for the conference attendees, each attendee will be billed by the hotel for his or her room, as well as any individual charges (meals, movies, etc.). Quotes shall be submitted via e-mail to the Contracting Officer and Contract Specialist identified herein. Quotes shall be submitted not later than 1:00 PM Local San Antonio Texas time (CST) on Wednesday, 7 Jan 2015. The Government anticipates awarding a single contract (See Attachment 2 - Pricing Schedule) on or about 9 January 2015. See attached Combined Synopsis-RFQ for complete notice.
