- THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought notice for the identification of potential vendors to provide an OMNIGUIDE LASER/W operator to the William S. Middleton VA Hospital. Contractor must provide CO2 OmniGuide Laser/10160, Omniguide ENT - Elevate Fiber/10821, OmniGuide Hand Piece set for ENT/10825 - Elevate as well as a service provider to operate the laser during the surgical procedure which is scheduled on 1/22/2015. The equipment must arrive in working order by 0700 the day of surgery in order to be checked over by the hospital biomed department. Any equipment requiring sterilization must arrive 24-48 hours prior to the case. Laser technician must be qualified and competent to operate the CO2 OmniGuide laser during surgical procedure. The purpose of this notice is to gain knowledge of potential qualified sources and their business size classification relative to NAICS code 532490. Consistent with the VA's Veterans First Contracting Program, VA is particularly interested in identifying Service-Disabled Veteran-Owned Small Businesses and/or Veteran-Owned Small Businesses with the capability to perform. All Contractors, are required to register in the System for Award Management (SAM) database before an award. Contractors are responsible for the accuracy of information added to SAM. To register, go to http://www.sam.gov/. Service-Disabled Veteran-Owned Small Businesses and Veteran-Owned Small Businesses should also indicate whether the company is verified by the VA in the Vendor Information Pages (VIP) Database at www.vetbiz.gov.
