Solicitation: 15-PH-P-0005a Agency/Office: Public Buildings Service (PBS) Location: Lower Manhattan Field Office (2PMSL) Title: Install and maintain bottle-less cold water fountain service for tenant agency. "Bottle-less Point-of-Use Water Coolers." Description(s): SOW dated: December 3, 2014 GSA Ref: #15-0023 Point of Contacts: GSA PM / COR: Ralph Archangel CO: Donna Mattos 12/3/2014 SCOPE OF WORK Project Name: Bottleless Point-of-Use Water Cooler Objective: Provide eleven (11) Bottleless Point-of-Use Water Cooler systems for monthly rental for quality drinkable water to a secure Federally-owned building. Period of Performance: One 12 month base period and four individual option periods consisting of twelve months each; if exercised by the government. Contractor will submit a proposal for the following: Base year: 12 months OY01: 12 months OY02: 12 months OY03: 12 months OY04: 12 months Total possible five year (5) contract; one (1) twelve mont base and four consecutive option years for twelve months each. Possible Project Start date: 1/2015 or 2/2015 1. Contractor will provide materials, labor, equipment and supplies to install eleven (11) new rental filtered water fountain units throughout the Silvio V. Mollo F.B. The purpose of Service includes providing portable electric coolers for tenant agency staff due to existing elevated levels of lead from New York City water supply system. 2. Contractor will provide the eleven (11) coolers and give continuous maintenance with service for the rented units coolers. 3. Coolers shall be portable and operate on a 120 volt electrical power source. The power source shall be furnished by the government. Coolers shall be capable of delivering cold water and shall be maintained in good working condition at all times. 4. The contractor must maintain the units; replace filters as needed (if they become unusable). In general, the cost of the annual filter change is included in the rental charge of the unit. 5. If a rental unit fails, to no fault of the Government, it is the responsibility of the contractor to replace the unit in a timely fashion at no cost to the Government. The contractor will service and maintain units; replace accordingly. 6. One of each new filtered water fountain unit will be installed at designated areas on each floor from the basement through ninth floors utilizing existing plumbing fixtures. 7. Each new water fountain unit will be furnished with filtration capability to mitigate adverse, bacteria; parasites and lead content to ensure dispensing water meets the minimum EPA established (ppm) threshold level for safe consumable drinking water. 8. Contractor will submit proposal for full installation and maintenance of each of eleven (11) fountain units for a full year period of Contract Term including but not limited to necessary filter changes, leaks in external connections (fountain assembly) and broken or damaged parts where contractor will respond on an as needed basis. Quantity subject to change. Considerations: 1. The contractor shall not disclose any information concerning this project to any entity except GSA. The contractor shall direct any inquiries to the Contracting Officer. 2. Presently, "MONTHLY" invoices will be submitted to: PBS Payment Branch, P.O. Box 17181, Fort Worth, TX 76102. Or you may submit an electronic invoice through the following website:www.finance.gsa.gov. You may access your payment information at http://www.finance.gsa.gov or Customer Service at (800) 676-3690. 3. SAM, formally: Contractor Registration (CCR) registration with the SAM System is required:www.SAM.gov. If your information in the SAM database is incorrect or expired, no payment will occur until the correct information is entered into the SAM database. The apparent winner must be registered and active status in SAM. 4. The government may terminate or end a contract at any time whether it is for the convenience of the government or due to fault by the contractor. 5. Respondents are solely responsible for all expenses associated to this Solicitation. 6. Responses to this Solicitation will not be returned. 7. The government is not responsible for lost or late proposals. 8. This procurement is for a firm fixed price contract. 9. This award will be made to the responsible bidder offering the lowest price and "Preference given through a local area set-aside to the New York City area." 10. GSA is an agency of the United States of America. As such, it is immune from federal taxes, as well as taxation by state and local governments. Require no deposits of coolers. 11: Vendors responding to this solicitation shall provide the following information: a. Provide a description of the services requested. b. Cost proposal, breakdown into BASE YEAR, OPTION YEAR 01, OPTION YEAR 02, OPTION YEAR 03, AND OPTION YEAR 04. c. The apparent winner maybe asked to provide further breakdown of all costs. d. GSA Schedule Number (if applicable) e. Name and number of a company representative in case we have additional questions. f. Business Size g. Dun & Bradstreet Number NOTE: 12. The government may unilaterally terminate all or part of this contract for convenience. Termination for convenience does not arise from any fault on the part of the contractor, but protects the government's interests by allowing it to cancel contracts for products that become obsolete or unnecessary.
