Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-79. FAR/VAAR clauses and provisions are available electronically respectively at http://www.acquisition.gov/far/index.html (FAR) and http://www.va.gov/oal/library/vaar/ (VAAR). This is a Request for Quotes under VA259-15-AP-1254 and only written responses are being accepted. This request is for a one time purchase. This is a total small business set-aside. The North American Industry Classification System (NAICS) Code is 339112, and the size standard is 500 employees or less. Requirement: The contractor shall furnish and deliver three(3) BladderScan BVI 9400 instrument along with three(3) BVI 9400 Rolling Carts. Dates and Place of Delivery: Delivery that is specified is to take place on or before FEB 13, 2015. The destination is the VA Eastern Colorado Health Care System, 1055 Clermont Street, Denver, CO 80220-3808 ITEM NUMBERDESCRIPTION OF SUPPLIES/SERVICESQUANTITYUNITUNIT PRICEAMOUNT 1 2 BLADDERSCAN BVI 9400 NONINVASIVE BLADDER VOLUME INSTRUMENT, TWO BATTERIES, BATTERY CHARGER, AND PRINTER PAPER. The offeror shall submit, with the offer, the name and address of the company supplying the instrument as well as a representative for direct contact.. Provisions, Contract Terms and Conditions: The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial items, does apply to this acquisition. This acquisition will be evaluated by Lowest Price Technically Acceptable (LPTA). The provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, applies.
