Description
Amendment 0001 for W91ZLK-15-T-0044 is issued to provide answers to Questions. The closing date of the solicitation remains the same at January 9, 2015 @ 10:00AM Eastern Time (ET). In accordance with the solicitation questions are to be received no later than January 6, 2015 @ 10:00 AM Eastern Time (ET). Questions received after this date may or may not be answered. No telephone requests will be honored. All terms, conditions, requirements and specifications of the original solicitation and Attachment 1 remain the same. Question 1: The descriptions do not line up with the part numbers. Starting with the 3rd item, the description to match is actually with the next part number below it. Would you please verify if the brand specific is marked accurately, and if it goes with the part number or the description? Answer 1: This Amendment 0001 is reposting the Attachment 2 Supply List, which is now in an excel format in lieu of the PDF format. This REVISED Attachment 2 ensures that the part numbers and descriptions are line up accurately and replaces the previous Attachment 2. The PDF Attachment 2 is canceled. Also per Attachment 1 Specifications Laboratory Supplies: the following still applies NOTE TO OFFEROR: It is requested that the offeror complete the unit pricing information for each line item on Non-Chemicals Requirement List Attachment 2. The estimated quantities for Non-Chemicals Requirement List Attachment 2 will be one (1) each and are intended for evaluation purposes only. These quantities may increase or decrease pending on government requirements, but will not exceed the total estimated amount of the order. Delivery is to be DESTINATION. Offers shall note the lead time, or approximate delivery date for items offered. ------------------------------------------------------------------------------------ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The combined synopsis/ solicitation number is W91ZLK-15-T-0044. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-79. The Government contemplates award of a Firm-Fixed Price (FFP) Purchase Order for laboratory consumable supplies, needed to sustain the basic operation of the United States Army, Public Health Command (USAPHC) laboratory in accordance with the two attachments: Specifications Laboratory Supplies (Attachment 1) and Non-Chemicals Requirement list (Attachment 2). This solicitation is set aside for small business. The Associated North America Industry Classification Code (NAICS) is 334516 and the Business Size Standard is 500. This acquisition is in accordance with FAR Part 13 and FAR Part 13.5 Test Program for Certain Commercial Items. This requirement is not being procured in accordance with FAR Part 8 and only Open Market, NOT General Service Administration (GSA) quotations are being accepted from responsible contractors. Acceptance and inspection shall be at DESTINATION. Final completion/destination shall be at USAPHC, ATTN: MCHB-TS-LOD/CSA, Bush River Road, Building E-2100, Gunpowder MD 21010-5403. CLIN 0001: The USAPHC has a requirement for laboratory consumable supplies to support mission critical needs to include field monitoring, preparation of quality control samples and analysis of environmental and occupational health samples. The USAPHC laboratory accreditation requires the uses of an approved manufactured list when procuring laboratory supplies. The supplies marked by an asterisk * shall be Brand Name in accordance with the attached requirements specified in the Non-Chemicals Requirement List Attachment 2. The supplies required must work with the existing lab equipment and meet the criteria of a quality critical supply for the USAPHC. Also the required brand names marked by an asterisk * have been reviewed, tested and acceptable for use within the USAPHC laboratory for the processing and analysis of laboratory samples. Using another brand name will invalidate the quality system and jeopardize the laboratories accreditations. The other listed items required which are not marked by an asterisk * shall be Brand Name or Equal. Purchasing of the laboratory supplies shall be in accordance with this solicitation, the attached Specifications Laboratory Supplies Attachment 1 and Non-Chemicals Requirement List Attachment 2. The award will be made on the basis of the lowest evaluated price of quotations meeting or exceeding the acceptability standards for non-cost factors, which are the technical requirements in accordance with attached W91ZLK-15-T-0044 Specifications Laboratory Supplies Attachment 1 and Non-Chemicals Requirement List Attachment 2. The technical evaluation will be a determination based on information furnished by the vendor. The Government is not responsible for locating or securing any information which is not identified in the quotation. The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable: a. Award will be made on the basis of the lowest evaluated price quotation technically meeting or exceeding the acceptability standards of the required laboratory supplies in accordance with the attached this solicitation, W91ZLK-15-T-0044 Specifications Laboratory Supplies Attachment 1 and Non-Chemicals Requirement List Attachment 2. The Government reserves the right to make an award without discussions. Government reserves the right to award on all or none basis. b. Past performance will also be evaluated, the offeror shall propose Laboratory Supplies which have been manufactured and sold for at least 1 year.
