Description
STATEMENT OF WORK 508-15-1-5103-0062 A.GENERAL INFORMATION 1. Title of Project: Specialty Functional magnetic resonance imaging (fMRI), magnetic resonance imaging (MRI), magnetic resonance spectroscopy (MRS), and positron emission tomography (PET) Services for the VA Rehabilitation Research &Development Center for Visual and Neurocognitive Rehabilitation's Research Projects 2. Scope of Work: The VA RR&D Center for Visual and Neurocognitive Rehabilitation (CVNR) has multiple funded research projects that require fMRI scanning services and specialized companion equipment and software to successfully carry out their research protocols. The objective is to acquire the use of this specialized equipment and services to operate the equipment, and to have this facility within 20 minutes of the Atlanta VA Medical Center. The research participants will need to commute between the scanning facility and the VA research labs frequently. 3. Background: This is a contractual service agreement that has historically been between VA Rehabilitation Research and Development Center of Excellence and the Emory University for the use of 3 Tesla (3T) Siemen's MRI scans. VA Central Office has funded the following Research Projects which require the services in this contract: "Grant # C9246C "Rehabilitation Research and Development Center of Excellence Grant" ;PI: Krish Sathian, MD, PhD (multiple projects and pilot projects) "Grant # B7676P "Brain Activations in Humans"; PI: Andrew Butler, PhD "Grant # B0994P "Right Frontal Activity in Older Adults: Does It Help or Hurt Word Retrieval?"; PI: Bruce Crosson, PhD "Grant # B8034 "Aerobic Exercise and Cognitive Training in Older Adults"; PI: Joe Nocera, PhD "Grant # E0596W "Effects of Exercise Intervention on Aging Related Motor Decline (AGING)"; PI: Keith McGregor, PhD "Grant # N0870W "Optimizing Motor Training in Parkinson Disease through Neural Mechanisms"; PI: Madeleine Hackney, PhD "Any other projects for CVNR investigators and/or career development awardees that are funded or may become funded during the period of this contract. Data Management: The contractor will perform MRI scans on research subjects. Each research subject will be issued, by the VA research staff, a code which will be entered into the scanner computer instead of a name. The key to the code is kept locked in a filing cabinet the locked office of the Principal Investigator. In addition, the "date of birth" appears in the scanner program as well. To avoid using the research participants actual date of birth, 1/1 for the month and day, are entered and the year of the birth of the subject. The MRI scanner automatically imprints the date of the scan on each MRI image. After the scan is complete, the images are transferred from the CSI MRI secured server to VA owned server, using FIPS 140-2 compliant encryption and password protected servers in secure data centers in use by Emory University. The data centers offer restricted access and are monitored 24 hours per day, 7 days per week, per agreement between Emory University and the Atlanta VA Medical Center. All data are stored on these servers, and data are accessed for analysis by workstations at the Atlanta VA Medical center, in the CVNR Neuroimaging Core in room 6205 in the Woodruff Memorial Research Building or PI's laboratory at Emory University in the Rehab Medicine Building, Room 150, 1441 Clifton Rd. Atlanta, GA 30029. Upon transfer of images to the VA servers, all scans are given the same date so that they cannot be identified by date of scan. 4. Performance Period: [Firm-Fixed-Price] The contractor shall complete the work required under this SOW within 1 year or less from date of award, unless otherwise directed by the Contracting Officer (CO) or the option years are exercised. If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. The work shall begin within 1 calendar day of award, unless otherwise specified. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. Base Year: 365 days Option Year 1: End of base year + 365 days Option Year 2: End of option Year 1 + 365 days Option Year 3: End of option year 2 + 365 days 5. Type of Contract: This is a firm-fixed price contract B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. 2. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. 3. The contractor must be located within 20 minutes of the Atlanta VA Medical Center at 1670 Clairmont Road, Decatur, GA 30033. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. Task One: The contractor shall provide the equipment necessary to complete the scanning to the specifications needed for the research projects. Deliverable One: The contractor shall provide the use of specific MRI equipment needed for the success of the research projects which include: a)Siemens Medical Solutions 3.0 Tesla Trio MRI scanner b)Avotec Silent Scan 3300 audio system c)Sensimetrics S14 sound system d)Current Designs fiber optic response system (fORP 932) with 2 4-button response boxes e)SR Research Eyelink 1000 Plus eye tracker f)Biopac psychophysiology recording suite g)PST vision correction lenses (+/- 2 to +/-7) h)Sony Data Projector (VPL-FH35) and custom display screen i)Mock Scanner with audio and visual capabilities The contractor shall maintain this equipment and make sure it is in working order. This includes upkeep of all manufacturer recommended maintenance and calibrations. The contractor shall notify the VA Principal Investigator and the COR within 24 hours if an equipment break-down occurs and will keep the VA personnel informed as to the progress and resolution of the problem. Task Two: The contractor shall make available to the VA personnel the ability schedule appointments for VA research participants. Deliverable Two: VA personnel shall have access to the contractor's current scheduling system. They will be able to make appointments for VA research subjects based on the availability of the scanner. The VA appointments will be viewed with the same importance and urgency as any other institution using the scanning equipment. Appointment confirmation will be delivered within one business day of making the appointment. Time conflicts with other parties that arise within this intermediate confirmation period shall be announced to the VA investigator immediately upon detection. Further, the contractor shall make reasonable attempts to accommodate that VA investigator's session at an alternate time should conflict with a third-party's scanning session arise. Task Three: The contractor shall provide qualified personnel to perform a scanning safety check prior to each scan. Deliverable Three: The contractor shall ensure that all an authorizedMRI technologist will perform a scanner safety check independent of the VA investigators. They will also provide safety training to all users. Task Four: The contractor shall provide qualified personnel to operate the scanners and accessory equipment (unless the VA investigator has obtained the proper permissions and training to run the scanner unattended during non-business hours). Deliverable Four: The MRI technician shall be preparing the scanner environment to allow rapid and comfortable placement of the participant into the MRI scanner to acquire study data. The technician shall ensure proper head placement of the study participant into the scanner bore, as also meets the consent of the participant for comfort. The technician shall ensure all necessary scanning coils are connected and functioning prior to placement of the study participant. In cases where a VA investigator needs to access the scanner after business hours, they will be allowed to do so if they have been pre-approved and trained to run the scanner without facility personnel and if the schedule allows. Task Five: The contractor shall provide a secure server to collect the scanning data and allow the data to then be transferred via secure encrypted connection onto a VA owned server at the PIs request. Deliverable Five: The contractor shall place data only on the secure Siemens Syngo scanner control computer for immediate transfer via encrypted tunnel to VA-owned servers managed by Emory University Technology Services. The Siemens Syngo computer must prevent third-party users from unauthorized access to scanning data using best practices of computer systems administration. The contractor shall maintain this equipment and make sure it is in working order. This includes upkeep of all manufacturer recommended maintenance and calibrations. The contractor shall notify the VA Principal Investigator and the COR within 24 hours if an equipment break-down occurs and will keep the VA personnel informed as to the progress and resolution of the problem. Fee Schedule for Scans: fMRI Scan Time with assistance: $450.00 per hour fMRI Scan Time without assistance: $350.00 per hour QUALITY ASSURANCE: PERFORMANCE STANDARDS REQUIRED SERVICEPERFORMANCE STANDARDMONITORING METHODINCENTIVE/DISINCENTIVES FOR MEETING OR NOT MEETING THE PERFORMANCE STANDARDS Provide the equipment necessary to complete the scanning to the specifications needed for the research projects.All equipment listed is available to the VA staff (after appointment is scheduled) and in working order 95% of the timePI and VA staff observation and communication from contractor.VA will not deliver payment for any scheduled scans that were not able to be completed or completed correctly because of equipment malfunction or break down. Notify COR and PI of any equipment malfunctions or break-downsReport to the VA personnel within 24 hours of breakdownPI and VA staff observation and communication from contractor.Notify the VA of within 24-72 hours will result in a past performance write up after the conclusion of the contract. Notify the VA after 72 hours may result in not exercising remaining option years on the contract Make available to the VA personnel the ability schedule appointments for VA research participantsAppointment scheduling system available to VA employees 95% of the timePI and VA staff feedback to COR on a quarterly basis or immediately when an emergent problem occursAppointment system not being available to VA employees 85%-95% of the time will result in a past performance write up after the conclusion of the contract. VA employees not being able to access less than 85% may result in not exercising remaining option years on the contract Provide qualified technicians to operate the scanner and accessory equipment. A qualified scanner operator must be present at each scanning session 100% of the timePI and VA staff feedback to CORFailure to provide qualified technicians for appointments more than twice can result in cancellation of the current contract and concurrent option years. This is a critical safety issue. E. EVALUATED OPTIONAL TASKS AND ASSOCIATED DELIVERABLES [If applicable] None F. SCHEDULE FOR DELIVERABLES 1.The contractor shall complete the Delivery Date column in Attachment A for each deliverable specified. 2. If for any reason any deliverable cannot be delivered within the scheduled time frame, the contractor is required to explain why in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response, in accordance with applicable regulations. G. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. H. REPORTING REQUIREMENTS A progress report shall cover all work completed during the preceding month. This report shall also identify any problems that arose, along with a statement explaining how the problem was resolved. This report shall also identify any problems that have arisen but have not been completely resolved, with an explanation. I. TRAVEL [If applicable] None J. GOVERNMENT RESPONSIBILITIES [If applicable] The government will provide: a)VA personnel to escort and monitor all data collections in the scanner with each research participant. b)VA personnel to schedule all appointments in the scanner and cancel them if needed with contractor's personnel. c)VA servers to transfer the collected data. K. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this contract, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the contract. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request for information relating to this contract, presented to the contractor, shall be submitted to the CO for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. M.CONTRACTOR PERSONNEL SECURITY REQUIREMENTS No contractor employees shall have access to the Department of Veterans Affairs' computer systems
