- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number and a statement that the solicitation is issued as a request for quotation (RFQ). VA259-15-Q-0154 (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (iv) This notice is set aside for 100% small business with the associated NAICS code 423720 standard, 100 employees. (v) A list of contract line item number(s) and items, quantities and units of measure per attached RFQ (vi) This is for rental of industrial boiler to be delivered and set up per attached Statement of Work and specifications (vii) Items delivered 15 days After Receipt of Order and FOB destination. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition regarding any addenda to the provision. Statement of Work Boiler #1 Rental November 24, 2014 Project Overview The Department of Veterans Affairs (VA) intends to procure services of a qualified vendor to furnish and place a temporary fire tube boiler at the Grand Junction Veterans Health Care Center (GJVHCC) located at 2121 North Avenue, Grand Junction, Colorado. See enclosed drawings of the Grand Junction Veterans Affairs (VA) campus map and specific bid location. Background The VA is pursuing this contract due to the failure of Boiler #1 fire tubes to meet inspection criteria of the State of Colorado. Failure to operate Boiler #1 creates risk to the mission of direct patient care during the winter months with the failure of Boiler #1, the Health Care Center no longer has back-up contingency. It is imperative that Boiler #1 be replaced immediately with a temporary boiler to assure redundancy can be met for the upcoming winter season. At the present time, if boiler #2 was taken out of commission due to maintenance or malfunctions during outside temperatures of 32 deg. F or less, the Boiler Plant would not be sufficient to handle the load and would put the GJHCC in a critical situation for patients and crucial services such as SPS, OR, Nutrition, patient hot water, heat, laundry and for medical waste disposal, etc. Description of Work for Boiler #1 Rental This work includes specifically the list of bid items that follow this general description of work for the Boiler Plant located in Building 9. This statement requires the vendor to furnish and place a portable steam boiler according to the following specifications: One (1) year rental with a two (2) year option. Minimum 200 BHP at 6900 PPH 105 # delivery steam Natural gas fired at 10psi No. 2 diesel 1 ½" pumped supply 480vac power available Softened water available Boiler feed water available Placement of unit: within -50'- of Building 9 Operation instruction and training required Chemical additives determined Boiler plant control system is ABB. All work will comply with VA Boiler Safety and Safety Specifications, ASME Standards and must meet BEI Requirements. Base Bid: MODULAR BOILER This bid will require construction services, delivery, installation and startup of a 9 PPM low NOX fire tube modular boiler with the ability to generate a minimum 6,900 lb/hr (200 Hp) with the ability to run on natural gas as the main source of fuel and No.2 diesel as a secondary fuel. Contractor is to provide required emissions information with the submittals. The temporary boiler must be placed in an area adjacent to the existing steam generation plant. Connection of steam supply, feed water, natural gas fuel, No. 2 diesel fuel and electrical power shall NOT be a part of this scope. Vendor shall cooperate with the owner (VA) providing connection the existing facility. No structural modifications of the building or control room architectural modifications are intended in this contract. VESSEL INSURANCE AND TESTING CERTIFICATION The Contractor shall provide the Government with an original, signed Accord Certificate of Insurance as evidence of having an active Pressure Vessel Insurance policy in effect with a policy limit of a minimum of $5 million naming the Grand Junction Veterans Health Care Center as an additional insured within 7 days after contract award. The Contractor shall ensure that this Pressure Vessel Insurance policy period will remain in effect for a minimum of one year after Government acceptance of all work required under this contract. The contractor shall provide to the Government a current boiler operational permit.
