REQUEST FOR QUOTATION: This requirement is for National Emissions Standard for Hazardous Air Pollutants (NESHAP) Asbestos Survey and Testing at the Bay Pines VA Medical Facility, 10000 Bay Pines Blvd, Bay Pines, FL 33744 per attached 1449 solicitation document VA248-15-Q-0477. This is a 100% Small Business Set-aside under NAICS Code 541620 with a $15M Size Standard. Delivery Information: Delivery is required no later than 3/1/2015. SITE VISIT: A Site Visit times will be held on Wednesday, 1/14/2015, at 9:00AM EST at the C.W. Bill Young VA Medical Center, 10000 Bay Pines Blvd, Bldg. 24, Bay Pines, FL 33744. Attendance to the Site Visit is highly encouraged and expected prior to submission of a quote for this project. Please contact Ms. Margaret Griffin by e-mail at margaret.griffin@va.gov to sign-up for a Site Visit no later than Tuesday, 1/13/2015, 2:00PM. When submitting your request, please courtesy copy Steven Scharlow at steven.scharlow@va.gov. Contractors shall meet Ms. Griffin on the 3rd floor, Conference Room 327, in Bldg. 24. Ms. Griffin may be reached on her cell at 727-432-5230. Vendors are required to fill out and submit the attached New Vendor Request Form with their quotes. Method of payment will be Electronic Payment by Electronic Funds Transfer. Upon completion of work all Invoices from the contractor shall be submitted electronically in accordance with VAAR Clause 852.232-72 Electronic Submission of Payment Requests. Quotes must represent best value and result in the lowest overall cost alternative (including technical acceptable) in accordance with the requirements and delivery. The agency will award a Firm-Fixed Price (FFP) contract using the LOWEST PRICE TECHNICALLY ACCEPTABLE (LPTA) methodology. The Government reserves the right to make award without discussions. Please return a signed & dated 1449 solicitation document VA248-15-Q-0477 with the pricing filled in for CLIN provide a price breakout for equipment, supplies, and services with your quote. Note that services shall be broken out by labor category, fully burdened labor rate, and estimated hours. The resultant contract will be a Firm Fixed Price (FFP) award. Note that interested vendors should review the current SCA Wage Determination to insure wage rates are at or above the minimum required amount. To receive an award, contractor must be registered in the System For Award Management (SAM) database (www.sam.gov). VENDOR QUESTIONS: After the Site Visit, All vendor questions concerning this requirement must be submitted electronically no later than 3:00 PM EST, Friday, 1/16/2015.
