Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or 4) Large business firms, to provide for the Captain James A. Lovell Federal Health Care Center (FHCC), North Chicago, IL, the lease of a Mobile Laboratory Trailer as described in the summarized Statement of Work (SOW). Summarized Statement of Work: The Contractor shall furnish a Mobile Laboratory Trailer outfitted to allow movement of identified Histology, Cytology and Microbiology staff, equipment, phones and computers into the trailer to continue operation of these services during renovations. This contract will also cover any maintenance support services, delivery and positioning, assistance with connections (electrical, water and sewer), training, set-up and removal. The trailer must meet all minimum applicable CLSI guidelines when it comes to design such as: Health and safety, biohazards, space determinations, finishes, ventilation, wall construction, electrical and communications, lighting and plumbing. This information can be found in CLSI document QMS04-A2, Laboratory Design; Approved Guideline-Second Edition. The vendor/trailer must also meet all applicable Design and Construction Standards. This information can be found in the U.S Department of Veterans Affairs Technical Information Library (TIL). All work shall be done in accordance with all current NEC, UL, OSHA, JCAHO, NFPA, federal, local, state and national building codes. Trailer Specifications: Width: The unit must be capable of expanding above grade level in order to clear a curb. This applies if the unit had a width greater than 18'. The trailer must have a width less than 25 ft. Space requirements include trailer and handicapped accessible ramps and stairs. Length: The length must be greater than 50 ft. and less than 105 ft. Space should include trailer and handicapped accessible ramps and stairs. Square Footage: The total square footage of workable laboratory space must be greater than 1100 square feet. The trailer laboratory design must be modifiable. Upon contract award the vendor will design, with the FHCC laboratory staff input and guidance, a layout consisting of but not limited to countertops, clean and dirty sinks, hoods, electrical hookups for equipment, leg wells, drawers and cabinets. This design will incorporate all FHCC laboratory processes and equipment (with specifications) to create an ideal working laboratory for the staff. Multiple revisions may be required. Trailer HVAC: The trailer must be equipped with an HVAC system that can stabilize the temperature and humidity inside the trailer between 59-77o F and 20%-60%. Air change rates must, at minimum, meet OSHA standards which are 10 to 12 outside air changes per hour. Trailer Chemical Fume Hoods (CFH) and Biological Safety Cabinets (BSC): Each trailer must contain one level 2 biological safety cabinet and one chemical fume hood with at minimum 9x6x6 inch, at maximum 12x8x6 inch sink enclosed within the fume hood. Each hood will need to have lighting on the inside of the hood. The biological safety cabinets (BSC) must contain HEPA filters. The Fume hoods must contain filters that prevent toxic, flammable or carcinogenic vapors from entering the ambient air, specifically Xylene and Formaldehyde. The biological safety cabinet must have a face velocity of 60-150 Linear Feet per Minute (LFPM). The chemical fume hood must have a face velocity of 100-150 LFPM. Information Resource Management (IRM) Requirements: A lockable, ventilated cabinet will need to be provided where the patch panel will be installed (by vendor) for all the network cable runs in the trailer will start from, the cabinet size needs to accommodate the patch panel, 1 network switch and a UPS. There also needs to be a power outlet in the cabinet. A 1.5 to 2 inch penetration (this will be used for the fiber and phone lines) from the cabinet to the outside of the trailer, on the same side and location as the power will be entering the trailer. This needs to be weather tight. This requirement will be awarded on a lowest-price technically acceptable basis. Tentative period of performance is for a one-year base period (estimated start of 6/1/2015), plus eight (8) 3-month option periods, to be exercised at the Governments discretion. All interested firms who can meet the requirements stated above should respond, in writing, with: 1.Company name. 2.Address. 3.Point of contact (name/telephone number/e-mail address). 4.DUNS number. 5.Tax ID number. 6.Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. 7.A capability statement that addresses the organizations qualifications and ability to perform as a contractor for the requirement depicted in the SOW. Submit responses to the Contract Specialist, Thomas Chesak, at thomas.chesak@va.gov no later than 11:00 a.m. (Central Standard Time), 12 January 2015. Mobile Laboratory Trailer Lease should be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 621511, Medical Laboratories, is applicable to this acquisition; the size standard is $30.0 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
