COMBINED SYNOPSIS/SOLICITATION FOR Cortrak 2 EAS Feeding Tube Systems This is a combined synopsis/solicitation for commercial items (supplies) prepared in accordance with the format at FAR Subpart 12.6., as supplemented with additional information included in this notice, and is being conducted pursuant to FAR Part Subpart 12.6 and FAR Part 13. The Request for Quote (RFQ) number is VA256-15-Q-0152. The synopsis/solicitation constitutes a RFQ and incorporates provisions and clauses in effect through Federal Acquisition Circular No. 2005-78 and Department of Veteran Affairs Acquisition Regulation Circular 2008-02. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The associated North American Industry Classification System (NAICS) code is 339112, which has a corresponding size standard of 500 employees. This is 100% set aside for small business. The Department of Veteran Affairs, Denver, CO VA Medical Center has a requirement for two (2) BRAND NAME or EQUAL Cortrak 2 External Access Systems for real time patient feeding tube insertion. IAW FAR 52.211-6, please provide the following if bidding an "or equal" product: A) Manufacturer's name B) Manufacturer's part number C) Description to include salient physical, functional, and or performance characteristics The following provisions apply to this acquisition: FAR 52.212-2 Evaluation - Commercial Items (Oct 2014): This provision is incorporated by reference, and paragraph (a) is modified to read as follows: (a) This is a competitive best value source selection in which the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price, and other factors considered. The Government intends to make award to the lowest priced technically acceptable offeror. The Government intends to award without discussions with respective offerors. The Government, however, reserves the right to conduct discussions if deemed in its best interest.
