Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. (ii) The solicitation number is VA251-15-Q-0125 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as full and open competition with a small business preference. The North American Industry Classification System (NAICS) code is 339113, and the size standard is 500 employees. (v) This requirement consists of six (6) line items: 1.Product # 85443 - (6 X 22mm) on 80cm catheter 6Fr Sheath - 1 EA 2.Product # 85453 - (6 X 22mm) on 120cm catheter 6Fr Sheath - 1 EA 3. 3.Product # 85407 - (8 X 59mm) on 80cm catheter 7Fr Sheath - 1 EA 4.Product # 85417 - (8 X 59mm) on 120cm catheter 7Fr Sheath - 1 EA 5.Product # 85414 - (7 X 38mm x 120cm catheter (7Fr Introducer 9 Fr Guide Catheter Compatible) - 1 EA 6.Product # 854157 - 7 X 59mm x 120cm catheter (7Fr Introducer 9 Fr Guide Catheter Compatible)- 1 EA NOTE: Offerors submitting quotations on Equal items shall provide the Manufacturer's Name, Brand and Part Number for each item providing a quotation on. (vi) To be technically acceptable the products quoted shall be Brand Name or Equal to the Atrium Medical stents listed above. (vii) Delivery FOB Destination to VA Medical Center, 4646 John R. Street, Detroit, MI 48201. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, FAR 52.209-5DEV; 52.225-25; 52.216-1 (fill in: Firm Fix Price contract will be awarded), 52.232-2, VAAR 852-215-70, 852.233-70; 852.233-71 (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov.
