- STATEMENT OF WORK TEMPLATE A.GENERAL INFORMATION 1.Title of Project 526-15-1-636-0071 Repair of broken waste pipe in street in front of the Medical Center 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified for the above project 2.Background Guardhouse toilet cannot be used as connecting waste pipe to city sewer is broken and must be replaced. 5.Performance Period: [Firm-Fixed-Price] The contractor shall complete the work required under this SOW in 30__ calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. 5. Type of Contract: [Firm-Fixed- STATEMENT OF WORK Please provide the following to the Bronx VA Medical Center: Replace waste pipe that connects guardhouse to sewer connection in the street on Webb Ave. Pipe has separated and must be replaced. Remove asphalt and dig trench for pipe from manhole cover to curb. All work below 5 feet will require shoring and sheathing. Replace 4 inch pipe as required for proper drainage, approximately 10-15 feet and reinsert into manhole. Replace earth and asphalt. Coordinate with City for any required permits.
