- Financial Institution Information Form Attachment 5 Past Performance Information Form Attachment 4 Offeror Schedule Attachment 3 Quality Assurance Surveillance PLan (QASP) Attachment 2 Performance Work Statement (PWS) Attachment 1 Solicitation FA4528-15-R-0001 The 5th Contracting Squadron is soliciting competitively for Packing and Crating Services at Minot AFB, North Dakota and surrounding areas. The contractor will furnish all personnel, equipment, facilities, supplies, services and materials for the preparation of personal property of Department of Defense (DOD) personnel for shipment and/or storage and related services through the Direct Procurement Method (DPM). AREA OF RESPONSIBILITY: Areas of Performance: Area 1, Zone 35: McHenry, McLean, Renville, Ward. Area 2, Zone 36: Burke, Divide, McKenzie, Mountrail, Williams. Area 3, Zone 38: Adams, Billings, Bowman, Dunn, Golden Valley, Grant, Hettinger, Mercer, Morton, Oliver, Sioux, Slope, Stark. Area 4, Zone 39: Benson, Bottineau, Pierce, Rolette, Sheridan, Towner, Wells. Area 5, Zone 40: Burleigh, Emmons, Kidder, Logan, McIntosh. The Contractor shall perform in accordance with standards which will be outlined in the posting of the forthcoming Performance Work Statement. A Firm-Fixed Price Performance-Based contract will result from the solicitation, with services to commence on or about 1 June 2015 (subject to the availability of funds). The performance period of this contract shall consist of one base year with four (4) option years. There is no guarantee that options will be exercised. This action is 100 percent set-aside for Small Business (SB) concerns. The applicable North American Industry Classification System (NAICS) code is 488991 with a small business size standard of $27,500,000. No paper copies will be issued. Potential offerors are responsible for monitoring this site for any updates/changes to the solicitation package by amendments. To be eligible for contract award, offerors must complete Offeror Representations and Certifications and complete vendor registration on the System for Award Management (SAM) website at https://www.sam.gov/portal/public/SAM/ prior to submitting an offer.
