Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. The purpose of this Sources Sought is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified sources. Interested contractors shall provide a capability statement that addresses the organizations qualifications and ability to provide thoracic surgical services in the Thoracic Clinic to cover a workload on average of four (4) hours per day, one day per week (Monday except on federal holidays). Estimated average workload is eight (8) patients per day. Scope of Duties Outpatient A minimum of one (1) physician shall provide thoracic surgical services at the Phoenix VA Medical Center in the Thoracic Clinic to cover a workload on average of four (4) hours a day, one day a week (Monday except on federal holidays). Estimated average workload is eight (8) patients per day. Emergency consults for thoracic surgery will be scheduled in the appropriate clinic or seen by a thoracic surgeon in a timely manner. All new patients to the clinic for which the attending physician is responsible will be seen by or discussed with the attending physician at the initial visit. This shall be documented in the medical record progress note by the attending physician or reflected in the resident's note to include the name of the attending physician and the nature of the discussion. Return patients shall be seen by the attending physician with such a frequency as to ensure that the course of treatment is effective and appropriate. This continuing involvement by the attending physician shall also be documented by the resident in the progress notes. Clinic schedules will be set at a level that all patients can be seen during scheduled clinic times. Physician shall be present for scheduled clinic times and complete all evaluations and reviews on the clinic day. Encounter forms shall be completed by physician at the time of clinic visit. Computerized notes shall be recorded on all visits. Attending physician shall be present in all clinics for the duration of the clinic. The attending physician shall provide notification 4-6 weeks in advance if clinic is to be canceled for non-emergent reasons. Inpatient The physician shall perform thoracic procedures and provide timely response to inpatient consultations at the Phoenix VA Medical Center. Surgical cases shall be coordinated with the Chief, Surgical Service at the Phoenix VA Medical Center. Staff rounds shall be made on a consistent basis to cover the complexity of Post-operative care and progress shall be reviewed with staff regularly and staff will be involved in any post-operative problems to the degree necessary. Discharge notes shall be dictated 24 hours prior to patient's discharge from the hospital and operative notes shall be dictated on the day of surgery or no later than 24 hours after surgery. Emergency Situations: Defined as a situation where immediate care is necessary to preserve the life of or to prevent serious impairment of the health of a patient. After appropriate notification to an attending physician of the emergency situation, the attending physician will determine where or not his/her physical presence is needed. If not, the attending physician shall see the patient and document concurrence of treatment rendered by the thoracic surgery resident in the computerized patient record system (CPRS) within 24 hours of the emergency situation. Physician shall complete medical record documentation and professional direction guidelines as described in VHA Handbook "Supervision of Residents." Documentation of attending physician supervision shall be done by progress notes entered into the computerized patient medical record at a frequency appropriate to the patient's condition. For patients admitted to inpatient services, the attending physician shall meet the patient within 24 hours of admission and personally document findings or concurrence with the resident's initial diagnosis and treatment plan as well as any modifications or additions. If a patient is admitted during the weekend or holiday for non-emergent care, a senior resident who is authorized to act as a teaching assistant may evaluate and discuss the patient's circumstances via telephone with the assigned attending physician. This discussion shall be documented in the progress notes. The attending physician shall then enter a confirming annotation the next working day. General Requirements: The attending physician who is responsible for consultations on specialty services shall either render the consultation or personally supervise the consultation by a resident within 24 hours of receipt. Documentation of this supervision shall be reflected in the medical record. The attending physician, in consultation with the resident, shall ensure that discharge or transfer of the patient from an inpatient setting or clinic includes specifics on physical activity, medications, diet, functional status and follow-up plans. At a minimum, this assurance shall be documented in the medical record through counter signature of the discharge summary or clinic discharge note. The attending physician shall assure, through documentation in the medical record, that all technically complex and high risk diagnostic and therapeutic procedures are medically indicated, fully explained to the patient, properly executed, correctly interpreted and evaluated for appropriate and effective follow-up. For elective or scheduled procedures, the attending physician shall evaluate the patient and write a pre-procedural note describing the findings, diagnosis, and plan for treatment and/or choice of specific procedure to be performed. The attending physician shall supervise modifications of treatment planning in response to the significant changes in diagnosis or patient status. Evidence of this involvement shall be documented in the progress notes. Residents may write "Do Not Resuscitate" orders only after consultation with the attending physician. The content of this discussion and the attending physician name shall be evidenced in the medical record, and the attending physician shall annotate the medical record within 24 hours. Requests for administration of anesthesia and for performance of operation and other procedures (informed consent) shall reflect the name of the staff practitioner performing and/or directing the performance of the procedure. Consent form shall be signed, dated, and timed by the patient prior to initiating the procedure with appropriate witness signature. The attending physician shall read and agree upon recommendations made in the discharge summary and shall authenticate the procedure/operative report. The attending physician shall sign discharge summaries, operative reports and all forms requiring attending signature to assure medical record completion as soon as possible, but no more than ten (10) calendar days after discharge. The attending physician shall complete peer reviews as requested. The attending physician shall attend and participate in semi-monthly Cancer and Tumor Boards (twice a month); Morbidity & Mortality conferences (twice monthly) and monthly Surgical Service staff meetings. As it is impossible to determine the exact workload that shall be required during the period of the contract, all thoracic surgery services that are required shall be performed by the thoracic surgeons as specified in the contract. The services specified shall only be changed by written modification to this contract by a warranted VA Contracting Officer. The VA reserves the right to make changes to this contract based on workload and internal staffing levels. The services to be performed by the attending physician shall be performed in accordance with VA policies and procedures and the regulations of the medical staff by-laws of the Phoenix VA Medical Center. Clinical coverage review shall include a description of the monitoring procedures used by the Phoenix VA Medical Center to ensure contract compliance which will be demonstrated through time and attendance logs, surgical room records, minutes of meetings, or other appropriate records, that the services called for under the contract have been received by the Contracting Officer's Representative (COR) who is responsible for verifying contract compliance. After contract award, any incidents of contractor non-compliance, as evidenced by monitoring procedures, shall be forwarded immediately to the contracting officer in writing. All contractor personnel (including students, trainees, research personnel and staff) working within the hospital, clinics, and/or nursing home care unit shall have completed the following mandatory screening and/or training prior to entering the VA premises: Screening for tuberculosis per OSHA regulations, Hepatitis B vaccination and blood borne pathogen training per OSHA regulations and Infection control and safety training per VA regulations. TERM OF CONTRACT The base period of performance will be effective one year from the date of award. The contract has four 1-year option periods that may be exercised by the VA. No services are to be performed after the expiration date of each contract period unless written authorization is furnished by the Contracting Officer. The contract is subject to the availability of funds. PERFORMANCE PERIOD Services hereunder shall cover the Base Period beginning on the date of award through twelve consecutive months, with the option to renew for four- one year periods. Base Period:April 1, 2015 through March 31, 2016 Option Year One:April 1, 2016 through March 31, 2017 Option Year Two:April 1, 2017 through March 31, 2018 Option Year Three:April 1, 2018 through March 31, 2019 Option Year Four:April 1, 2019 through March 31, 2020 Qualifications: Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. Each person assigned to work under this contract shall be licensed by a State, Territory or Commonwealth of the United States or the District of Columbia. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The North American Classification System (NAICS) code for this acquisition has been determined to be 621111 (Offices of Physicians (except Mental Health Specialists)) with a size standard of $11.0 Million. Responses to this Sources Sought must be in writing and submitted via email to carlos.jaquez@va.gov. Telephone responses will not be accepted.
