- Please complete pricing in the attached bid schedule. Return the bid schedule with your quote. This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This procurement is being issued as full and open competition. This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-78 effective 26 Dec 14 and DFARS Change Notice 20141216. The North American Industry Classification System (NAICS) code is 334510 and the size standard is 500 employees. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Since award will be based on initial responses, offerors are highly encouraged to quote their most advantageous pricing in their initial response. Award will be made in accordance with FAR Part 12, Acquisition of Commercial Items using Simplified Acquisition Procedures in FAR 13.106. REQUIREMENT/SALIENT CHARACTERISTICS: This is a brand name or equal requirement for the Benson CCA-200 Mini Audiometer System with delivery and installation of three systems at Joint Base San Antonio, Lackland, and delivery and installation of two systems at Maxwell AFB, AL to perform basic and officer trainee hearing exams. This requirement consists of an expansion audiometer booth mount for the audiometer to daisy chain to seven other mounts, a BAS-200 simulator bio-acoustic used for required daily calibration checks and BAS-200slm with sound level meter for ambient noise level monitoring. Training will be conducted by the vendor. Delivery/installation/training is needed as soon as possible. The system must be compatible with the Defense and Occupational Environmental Health Readiness System - Hearing Conservation Program (DOEHRS-HC). QUOTE REQUIREMENTS: Quotes should include delivery date, discount terms and DUNS number. Quotes must also be submitted on an all or none basis and contain product specifications if quoting on an "equal" product. Specifications shall meet all criteria listed in the item description. FOB Destination for delivery to Wilford Hall Ambulatory Surgical Center, 2200 Bergquest Ave, JBSA Lackland, TX 78236-5253 and to the 42d Medical Group, 300 S. Twining Stree, Bldg 760, Maxwell AFB, AL 36112. The government will award a contract resulting from this combo synopsis/solicitation to the lowest priced, responsible offeror who meets the requirement.
